Objective : To investigate plasma ghrelin levels in type 2 diabetes and diabetic ketoacidosis and its possible involvement in the regulation of metabolic balance. Methods :Plasma ghrelin concentration was measured by enzyme immunoassay in the controls and the patients with type 2 diabetes (T2DM) and diabetic ketoacidosis, plasma insulin levels were measured by radioimmunoassay (RIA), and plasma concentration of free fatty acids (FFA) were measured by spectrophotography. Results :Plasma glucose concentrations were significantly higher in T2DM group and diabetic ketoacidosis group than the controls group( P <0.01), and significantly higher in diabetic ketoacidosis group than T2DM group ( P < 0.01) , plasma ghrelin concentrations were significantly lower in T2DM group and diabetic ketoacidosis group than the controls group ( P <0.05), and significantly lower in diabetic ketoacidosis group than T2DM group ( P < 0.05 ) ,TG concentrations were significantly higher in T2DM group and diabetic ketoacidosis group than the controls ( P <0.05), plasma FFA concentrations were significantly higher in T2DM group and DKA group than the controls group(P <0.01), and significantly higher in diabetic ketoacidosis group than T2DM group ( P < 0.05) , HOMA-IR were significantly higher in T2DM and diabetic ketoacidosis than the controls group( P <0.05 ), and higher in diabetic ketoacidosis group than T2DM group(P <0.05). Pearson correlations analysis showed that plasma ghrelin level were negatively correlated with blood glucose and low density lipoprotein-cholesterol (r=-0.25, P<0.05 and r=-0.32, P<0.05 respectively )and positively correlated with insulin(r=0.21, P<0.05). Conclusion :These findings suggest a possible involvement of ghrelin in the regulation of glucose and lipid metabolism, and it might have some role in the abnormal metabolism of T2DM .
