Expression Of Integrins And Extracellular Matrix Proteins In Non-small Cell Lung Cancer: Correlation With Tumor Metastasis And Prognosis

Objective For patients with non-small cell lung cancer(NSCLC),the biggest threat to survival is metastasis. During metastasis cascade, tumor cells interact with extracellular matrix(ECM) through certain adhesion molecules such as integrins. The aim of this study was to analyze the distribution of the integrin α 5 β1 and ECM protein in a series of patients with NSCLC to assess their distribution and correlation with lymph node (LN) metastasis of NSCLC and the correlation with prognosis.Methods one hundred and nineteen patients with non-small cell lung cancer(NSCLC) from January 1997 to October 1998 were entered into the study. All subject were matched up with the inclusion criteria and followed up till June 2004.There were 30 patients with LN metastasis and 89 without LN metastasis. The expression of integrin α 5 β1 and collagen type IV,fibronectin, tenascin was studied by immunohistochemistry. Chi-Square tests were used to compare groups and parameters. The disease prognosis was analyzed by multivariate COX regression and Kaplan-Meier survival curves and Log rank tests.Results ?Positive rate of collagen type IV > fibronectinN tenascin, integrin a 5^ integrin P 1 inNSCLC was 78%^ 98%, 45%, 46% and 42%, respectively. The expression of collagen type IV and fibronectin were more frequently lost in LN metastasis group(P<0.05 ) ,and the expression of integrin a 5 and integrin P 1 was significantly associated with LN metastasis too, they have more expression in LN metastasis group(P<0.001 and P<0.001, respectively) ?Expression of the five group protein correlated with TNM stage> differentiation or lymph node metastasis (PO.05), but was not related to sex> age. Collagen type IV expressed more in earlier P-TNM stage and better differentiation, but integrin a5> PI was on the contrary.The expression of integrin a 5 was correlated positively with the expression of integrin P 1, and negatively with the expression of collagen type IV.(3)5-year survival rate of collagen type IV in positive group and negative group was 58.1 %and 11.5%, respectively, the difference between positive group and negative group was statistically significant (PO.001) . 5-year survival rate of integrin a 5 in positive group and negative group was 29.1% and 64.1%, the difference between the two groups was statistically significant (P<0.001) .Make the 73 stage I NSCLC on univariate analysis, the disease-free survival rate of patients with higher integrin a 5 expression was better than those with lower expression(P=0.034).Conclusions Increased expression of integrins a5, PI and lose expression of...
