Objective To investigate the impact of the variation in adiponectin gene on type 2 diabetes and insulin sensitivity in Chinese Hans. Methods The study population consisted of 409 Chinese Hans residents in Anhui province including 150 subjects with normal glucose tolerance (NGT) and subjects with 259 type 2 diabetes melluties (T2DM). The islet beta-cell insulin secretion and tissue insulin sensitivity was assessed by formulae of homeostasis model assessment(HOMA-IR,ISI & HOMA- β ). Height, weight, waist and hip circumferences were measured to calculate body mass index(BMI), Body Fat% and waist hip ratio(WHR). Firstly, Polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) was used to determine whether variation exist in apM1 gene. Then, exact variation was performed by automated DNA direct sequencing. Results (1) The HOMA-IR is associated with BMI(P<0.001),WHR(P=0.026),Body Fat% (P=0.001),FPG(P<0.001), 2hPG (P<0.001), DBP(P=0.011), FINS(P<0.001) and 2hINS(P<0.001) in Chinese Han population. (2)There were 4 variations in the coding region of apMl:SNP45 in exon 2;H142P, R221S and H241P in exon 3.(3) The genotypes distribution of apM1 SNP45 were 0.596 TT, 0.355 TG and 0.050 GG and its major allele was T(frequency=0.773) in subjects with NGT. The distributions of genotypes and alleles of SNP45 both were significantly difference between NGT and T2DM groups(P=0.003&0.001).The SNP45 non-TT(TG+GG) genotype was association with increased risk of T2DM (OR= 2.035,95 %C\ 1.337-3.097,P=0.001).Three missense mutations including H142P, R221S and H241P were found in heterozygous form.
