Objective: To investigate the relation between the expression of S100A4 protein and lung carcinoma, lung postoperative prognostic and clinical significance.Methods: the documents of 98 cases were taken from the patients undergone lobectomy of lung because of lung carcinoma between April 1998 and April 2001, including various patho-grouping and clinical stage. Immunohistochemical staining(s-p method) for S100A4 protein in lung carcinoma was performed and analyzed by statistics. To identify relation between the expression of S100A4 protein and lung postoperative prognostic follow-up was paid to all patients in 36 months. Results: The total positive rates of S100A4 protein in NSCLC was 59. 2%, of which the positive rates of squamous cell carcinoma, adenocarcinoma, adenosquamous carcinoma and large cell lung cancer were 48.6%, 71.4%, 75.0% and 100%. respectively, there was significant difference in poor differentiation (P<0.05), lymph node metastasis (p<0.05), distant metastasis (p<0.05), III + IV stage and age (≤50) (P<0.05) . To study relationship between the expression of S100A4 protein and clinicopathological parameters in lung cancer we found that expression of S100A4 protein was closely related to lymph node metastasis, distant metastasis, in age, histology differentiation, p-TNM stage, but had little to do with primary tumor, location, sex and smoking history. The total positive rates of S100A4 protein in tumor-stroma was observed in 67.3%. By follow-up, we found that survival rate of patients in 3 years with positive S100A4 protein and negative S100A4 protein were34. 5 % and 75.0% respectively, with significant difference between the groups (P <0. 05), and that survival rate of patients in 3 years with positive S100A4 protein and negative S100A4 protein were46.9% and 53.8% in tumor-stroma ,respectively, with insignificant difference between the groups (P>0. 05). A multivariate analysis indicated that S100A4 protein in cell, in age, lymph node metastasis, distant metastasis, histology differentiation, p-TNM stage were significantly hazardous factors, and primary tumor, location,sex and smoking history were not independent prognostic factors which affected survival.Conclusion: Expression of S100A4 protein in NSCLC was closely related to invasion, differentiation, metastasis, p-TNM stage and prognosis. S100A4 protein might become prediction of tumor progression of disease and direct marker of clinical therapy.
