part ⅠExperimental study on the mechanisms for MODS induced by peritonitis in rats with immunosuppression Objective: To study the mechanisms for MODS induced by peritonitis in rats with immunosuppression. Methods :Sixty female SD rats divided into two groups, each group had 30 rats. They were established to control group and immunosuppression animal model. All rats were induced peritonitis by CLP. The function changes of liver and kidney, the serum concentration of TNF-αand IL-10 were detected before and after CLP. The apoptosis of immune cells in spleen were observed by electron microscope. Results: 1. All rats had functional lesion of liver and kidney after CLP, it turn worse and worse. The 24th hour after CLP, the group of immunosuppression occurred MODS, the group of control did not. 2. After CLP, the concentration of TNF-αwas higher than that of before CLP in two groups(p<0.01), the 3rd hour after CLP, it reached the peak value. All the time, the concentration of TNF-αof rats with immunosuppression were lower than that of the contro(lp<0.01).3. At the time of the 3rd,6th,12th hour after CLP, the concentration of IL-10 was higher than that of before CLP in two groups(p<0.01), the 3rd hour after CLP, it reached the peak value, the group of immunosuppression were lower than that of the control(p<0.05).The concentration of IL-10 were no difference between before CLP and the 24th hour after CLP in two groups(.p>0.05).4.After CLP, the ratio of TNF-αand IL-10 was higher than that of before CLP(p<0.01), the ratio of immunosuppression was lower than that of the control(p<0.01). 5. The 24th hour after CLP, a number of immune cells occurred apoptosis in spleen, rare in control. We could not see it before CLP. Conclusion: Peritonitis of rats with immunosuppression is easy to result in MODS. Compensatory anti-inflammatory response syndrome (CARS) may be the mechanism. All the time, the concentration of TNF-αhad no difference in the two groups. 3.After CLP, the concentration of IL-10 was higher than that of before CLP in two groups, the 3rd hour after CLP, it reached the peak value, there was no difference between before CLP and the 24th hour after CLP in the group of control(p>0.05). The concentration of IL-10 was no difference between before CLP and the 12th hour after CLP in the group of aminophylline, the 24th hour after CLP, it was lower than that of before CLP(p<0.01). After CLP, the concentration of IL-10 in the group of aminophylline was lower than that of control(p<0.01). Conclusion: Aminophylline, the receptor antagonist of adenosine, can decrease the serum concentration of anti-inflammatory cytokine of IL-10 in immunosuppressed rats with peritonitis, and it is no effect on the pro-inflammatory cytokine of TNF-α. Aminophylline can delay the development of MODS in this rat.
