[OBJECTIVE]To research the extent , pattern and clinical significance of the histological inflammation and latent infection in the prostate samples of operational BPH patients ,and to explore the causes of the inflammation . Weare going to study the incidence of the histological inflammation and infection with aim to clear the possible reason of the inflammation. [OBJECTS AND METHODS]The study included 53 BPH patients admitting into our department during October ,2004 and December ,2004 for a operational therapy. They had the data of serum PSA concentration , urine laboratory examination and prostatic volume, and we evaluated the patients' symptom with IPSS, and them gained the EPS to test SIgA and complement c3 before TURP operation. During the operation, we took prostatic tissue sample to store under -70℃ for the bacterial 16SrDNA identification by the real-time PCR technique, paying attention to non-contamination, to measure the SIgA concentration in tissue and to certify and classify the histological inflammation. Besides, we examine the SIgA and C3 in the EPS of 10 normal yong men and bacterial 16SrDNA of 10 corporal prostates without inflammation in contrast to the patients. [RESULTS]Inflammation was identified in 98% patients, mainly existing in peri-gland tissue. The more severe inflammation was, the more concentration of C3 was, as is not the relation between IPSS and inflammation. The incidence of prostatic infection was 28.3%, and infection group had less SIgA concentration in both EPS and tissue than non-infection group significantly. Infection, but ,did not contributed to IPSS and maximum pressure and maximum closure urethral pressure .The prostatic infection group had bigger rate of inflammation than non-infection group. The elevation of serum PSA in BPH patients attributed to histological inflammation , increasing prostatevolume and age, excluding urine retention . There were more IPSS scores in small prostate volume group of patients(^30ml) than ones in big volume group(>30ml). The small prostate volume group was significantly more incident of bladder muscle damage or functional decline , which, however, had less maximum urethral pressure and maximum closure urethral pressure statistically contrasting to the bia prostatic volume groop. [CONCLUSION]Histological inflammation appeared in the prostate of the operational BPH patients,with a high rate of 98%, and prostatic infection incidence was 28.3% among such patients, but small prostatic volume group had no more infection rate and no more inflammation rate than big prostatic volume group. This inflammation may be asymptomatic. Local complement activation and inflammation in prostate of these patients were possiblly the important causes of the prostatic inflammation, which made serum PSA incline. Infection in prostate activated local complement system partially. Local SIgA decreasing was the precedence of prostate infection.
