Objective: Oral cancer as a common malignant tumors of the head andneck,its incidence has increased in recent years,and the lowing ages of onsetof the trend is more and more obvious.At present,in addition tooperation,radiation and chemotherapy,the new adjuvant therapy such asbiological treatment and targeted therapy,the cancer immunotherapy isconstantly used in clinical application.It makes a good effect in thedevelopment of cancer control.And it improves the patients' long-termsurvival rate and quality of life,it is the development trend of oral cancertreatment.Secretory immunoglobulin A(SIgA) is the main immunoglobulin inexocrine fluid.As the main effective factor in the process of mucosalresponse,it mainly through the inhibition of bacterial adhesion,making thetoxins and enzyme inactivation,neutralizing the virus,activating thecomplement system,mediating antibody dependent cell and mediatedcytotoxiccity to achieve the immune defense mechanism.It plays a major rolein the oral mucosa defense of the local immune.Interleukin6(IL-6) is aparacrine or autocrine cytokines,it can make B cells differentiation into plasmacells,which contribute to the antibody secretion,and its expression related withtumor formation and differentiation.Dendritic cell(DC) is the most effectiveantigen presenting cells in the body,its high level expression of MHC-class IImolecules can effectively uptake and process antigenic material and make thenaive T cells activation.Therefore,the DC has the important research value inorgan transplantation,autoimmune disease and tumor immunotherapy.This textresearch the content of SIgA,IL-6in the oral cancer patients' saliva and theexpression of DC in the tissue, analysis the relationship with the oral cancerclinical stage,pathologic type,differentiation degree,and presence of lymphnode metastasis;explore SIgA,IL-6and DC in oral cancer occurrence, development process function,relationship and clinical significance,learn localimmune status of oral cancer patients and tumor escape immunemechanism,for the prevention,diagnosis and treatment of oral cancer putsforward new ideas and new clinical strategies.Methods:60cases of surgical resection and confirmed by pathologyprimary cancer in our hospital dental form June2010to December2011hadbeen selected.Any anticancer therapy was not given before the operation,which were non acute infection period,excluding systemic diseases,including38cases of male patients,female patients in22cases;age range from26to70,the median age was52years old;I-II in patients with36cases,24cases ofpatients with stage Ⅲ-IV;with high differentiation29cases,31cases werepoorly differentiated;squamous cell carcinoma in40cases,20cases ofadenocarcinoma;lymph node metastases of17cases with positive,negative in43case.12cases of normal epithelium tissue confirmed by pathology were asnormal controls.Specimen are immediately after the materials,parted in twopieces,one pieces fixed in4%of formaldehyde together,paraffin embedding,paraffin line immunohistochemical detection;another one immediately preparesingle-celled leviatation liquid,flow cytometry (FCM) detection.Detect theexpression of dendrtic cells CD1a,CD83,CD80,CD86in oral cancer andnormal tissue respectively,analyze the relationship with tumor clinical andpathologic stage type,differentiation degree,and presence of lymph nodemetastasis.Collect the saliva of preoperative on the above60oral cancercases,20cases of healthy volunteers saliva as normal controls.All the selectedobjects don't have severe periodontitis and apparent oral mucosaldiseases.Collection of saliva samples immediately centrifugal(3000turn),taking on the liquid-80℃for preservation.Detect the saliva of SIgA,IL-6inoral cancer patients and normal controls with ELISA and analysis therelationship between various clinicopathological parameters.Reslut:1The content of SIgA in oral cancer patients' saliva obviously lower thanthe normal group,difference has statistical significance(P<0.05).The content of IL-6in oral cancer patients' saliva obviously higher than the normal group,difference has statistical significance(P<0.05).The content of SIgA and IL-6in saliva of oral cancer patients is irrelevant with the clinical,pathologic stagetype,histologic classification and lymph node metastasis(P>0.05).The contentof SIgA and IL-6in saliva is related with negative correlation (r=-0.993, P<0.05).2Flow cytometry test and immunohistochemical results show that theexpression of CD1a,CD83,CD80and CD86in oral cancer tissues wererespectively lower than normal tissue,the difference was statisticallysignificant(P<0.05).The expression of CD1a and CD83in oral cancer tissueis irrelevant with the clinical,pathologic stage type,histologic classificationand lymph node metastasis(P>0.05).The expression of CD80and CD86inoral cancer tissue is irrelevant with the histologic classification,pathologicstage type (P>0.05),but with the clinical stage and lymph node metastasis(P<0.05).The expression of CD80and CD86in I-II is higher than Ⅲ-IV,theexpression of CD80and CD86in lymph node metastases with positive islower than the negative(P<0.05).Correlation analysis showed that theexpression of CD80and CD86is negative correlation with clinical stage andlymph node metastases(P<0.05).Conclusion:1The content of SIgA in saliva of oral cancer patients is obviouslyreduce,whereas the content of IL-6is significantly increased,which can beused as auxiliary means of oral cancer diagnosis,with the correlation betweenthem,the increasing levels of IL-6may be one of the important reasons to leadto decrease SIgA generation,so it can be concluded that the oral mucosaimmune function impairment may be associated with the immune functionwith suppression.2The expression of CD1a,CD83,CD80and CD86in oral cancer tissuesignificantly reduce,which shows that DC in oral cancer patients existsimmune deficiencies,and the expression of CD80and CD86are relevant withclinical stage and lymph node metastasis,they can not only reflect the local immune status,but also prognostic evaluation.3The content of IL-6in saliva of oral cancer patients is rise,while thecontent of SIgA and expression of CD1a,CD83,CD80and CD86intissue,describe in the oral cancer microenvirment,IL-6may through inhibitionof SIgA formation,leading to formation of DC immune tolerance,therebyaffecting the immune response mechanism.
