OBJECTIVE: To study the feasibility and safety of serotonin (5-HT) reuptake inhibitor in the adjunctive therapy of the patients with OAB. METHODS: 43 patients with OAB were divided into trial group (n=23) and control group (n=20) for 4 weeks of therapy by single-blind and semi-randomized method. In the trial group, 5-HT reuptake inhibitor (20mg qd in the first 15 days and lOmg qd in the last days) and Tolterodine (5mg bid) were used together. In the control group, only Tolterodine was used 5mg bid. Based on the analysis of the frequency-volume chart and anxiety score (BAI), we studied the clinical efficacy, adverse effect and release of anxiety. RESULTS: The frequency-volume chart showed that the patients with OAB appeared to have various conditions of urination and there were different therapeutic effects achieved in different kinds of patients. In the trial group and the control group, the mean No. of micturition/24h were improved from 18.53±5.88 to 11.69±3.05 and 16.13±7.34 to 10.89±4.31 respectively. Meanwhile the mean volume voided per micturition/24h in the trial group was improved from 80.2±38.33ml to 123.77±61.19ml in contrast to 96.95±42.87ml to 136.33±61.01ml in the control group. Although the change of mean No. of micturition/24h and volume voided per micturition/24h in the two groups had statistic significance (P<0.001) after 4 weeks of therapy, there was no statistic significance between the two groups (P>0.05). In the trialgroup, the incidence of total adverse effect was 15.79% and most of them were dry mouth (10.53%). In the control group, the total adverse effect and the incidence of dry mouth both were 38.89%, accompanied by fidgety and dysuria (5.56%). There was no statistic significance between the two groups, P>0.05. The improvement of anxious emotion in the trial group was 81.82% in contrast to 22.22% in control group, which had statistic significance (PO.05). CONCLUSION: The frequency-volume chart plays an important role in the treatment of OAB. Clinician can obtain the real micturition situation with it. Thus they could make specific and effective treatment for OAB. Although further researches will be needed to confirm the feasibility of the clinical use of 5-HT reuptake inhibitor in the adjunctive therapy of OAB, the application of 5-HT reuptake inhibitor may have some therapeutic effects in the treatment of patients with severe dispersal frequency and can improve the emotional disorders of the patients with OAB without increasing the risk of side effect.
