Acute cerebral infarction is kind of frequently encountered and severe disease with high mortality and disability rate. Platelets play a critical role in arterial disease; thus, platelet activation is a most important pathophysical course in acute cerebral infarction. Platelets commonly keep inactivated when circulating in the vascular system, but under the circumstance of atherosclerosis, inflammation, hemorheological change, et al, the blood vessel endothelium destructed and collagen exposed, which activated platelet. When platelets activated and bound to fibrin and other formed elements in the vessel, thrombus formed. Seeking direct and quantified laboratory examination methods which indicate platelets activation is of great value in clinical work for it can provide simultaneous data of pathogenetic condition of different stage of the disease, improve antipltelet agents assessment and clinical antiplatelet therapeutical strategy as well. Study the platelet activation state in patients with acute cerebral infarction, offer clinical indicators, can help us appraise the medicine curative effect and monitor the patient's condition, and then guide clinical strategy. We gauge relevant parameters of platelet and make correlation analysis with CSS, at the same time, observe the effects of Ozagrel, then make clinical approach for anti-platelet medicine usage, and then offer theoretical foundation for treatment of acute cerebral infarction. We choose 145 patients with infarction of the cerebral median artery, whose onset duration were between 6~72 hours, from 2004.1~2004.12 at the department of neurology in China Japan Union Hospital of Jilin University, all patients accord with the diagnostic standard of the national fourth Meeting of cerebrovascular disease in 1996. Adopt automatic blood cell analysis instrument measure MPV, PLT and PDW of 145 patients, adopt FCM measure CD61and CD62p expression of 32, and measure CSS score of 145 patients at onset and 7,14 days. All patients accept antiplatelet treatment with Ozagrel. Observe its influence to relevant indicators of platelet, and make correlation analysis with CSS. The result shows, all patient's MPV, expression of CD61, CD62p increases and PLT reduces notably before intervention. After intervening, MPV, CD61, CD62p and CSS reduce significantly, PLT have no obvious change, compared with the PLT before treating there is not a statistics difference. Correlation analysis shows, MPV and PLT do not have statistics dependence to CSS, CD61 and CD62p is correlated with CSS positively. Platelet activation is one of important pathology foundations of acute cerebral infarction, platelet activation the increase the danger of thrombogenesis. This study shows, in acute state of ischemic infarction, platelet volume and some membrane glycoprotein expression increases and platelet count decreases, all the result reflects the activating of platelet, but the change of platelet count cause the danger of thrombus forming. In the different course of ischemic cerebral infarct, the main pathogenic factor is the activating extent of platelet but not its quantity, though above-mentioned indicators can reflect indirectly platelet-activating extent, but their specificity is relatively low, and can be interfered by other cells. The change of MPV and PLT that the research shows can reflect platelet-activating extent indirectly, but have no correlation with neurological impairment, vice versa CD 61 and CD62p can monitor patient's condition as laboratory index.
