| Objective: To investigate the effects of a new agent fasudil on delayed cerebral vasospasm after aneurysmal subarachnoid hemorrhage.Methods: Using Nimodipine as the control, with an open randomized control test, to study the vasodilative and neuroprotective function of fasudil. 35 eligible patients were randomized to divide into two groups, 13 cases in Fasudil group, 22 cases in Nimodipine group. Fasudil and Nimodipine were administered in double blind trail. Fasudil was infused intravenously every 8 hours, each time with 30mg infused within 30 minutes. Nimodipine was infused intravenously continuously for 24 hours with a dosage of 25mg. The agents were given successively for 14 days. Meantimes,clinical experience(including GCS,TCD,CT(Fisher score)Hunt-Hess,BP and HR before operation , before treatment and during 14 days,GOS of the last day of treatment and clinical presentation),blood examination (WBC, RBC, ALT, AST, BUN, Cr) should be performed during 14 days.Results: (1) Both Fasudil and Nimodipine have similar results in improving the prognosis of the patients. (2) Fasudil did not decrease the systemic blood pressure while Nimodipine decreased Bp obviously. (3) From side effect,Fasudil may be better than Nimodipine in the safety.Conclusion: (1) Fasudil has the same effect with Nimodipine in the prognosis of SAH patients. (2) Fasudil shows high selectivity for intracranial arteries compared with extracranial arteries and does not decrease systematic Bp.
