Aprotinin is a natural derivaive product obtained from bovine lung with an inhibitory effect on antifibrinolytic system,it's blood conservation effect has already been demonstrated by the world and was widely used in the open heart surgery.But the aprotinin also own the defects such as expensive price and anaphylactic reaction.so tranexamic acid appeared because of its low price and nonanaphylactic reaction.Tranexamic acid is a synthetic antifibrinolytic drug more recently introduced in cardiac surgery,but wheather its blood conservation function is as effective as or can replace aprotinin has not been generally acknowledged,still worth researching.OBJECTIVES:Compared tranexamic acid with aprotinin on their antifi--brinolytic,thrombotic,platelet function during CPB process and their blood loss,transfusion,explore their mechanisim.METHODS:82 patients undergoing cardiopulmonary bypass cardiac surgery (limited to adult patients, valve replacement surgery and coronary revascularisations) were randomized divided into AP.TA and control group. 36 patients received aprotinin, 350mg constant infusion after the induction of anesthesia but before the surgical incision, 350mg followed with the same way from the CPB curcuit after the beginning of the CPB. 30 patients received tranexamic acid, 0.75g directly injected into the vein after induction but before incision, 0.75g followed with the same way from the CPB curcuit after the CPB began.
