Objective: To investigate the effect of Color Doppler Flow Imaging (CDFI) and Color Doppler Energy (CED) in the Diagnosis of Budd-chiari syndrome (BCS) .Methods: A group of 70 patients suffering from BCS were examined with Method A (both CDFI and CDE) and Method B (CDFT only) respectively. All cases were proved by DSA and the operation .Results: Color Doppler Flow Imaging showed abnormal flow of HV and IVC. Spectral Doppler waveforms were displayed slowly , flat and continuously. By using blood flow diameter Color Doppler Energy showed a better accuracy in the representation of blood vessel diameters than Color Doppler Flow Imaging did, especially for the thick vessels, CDE blood flow diameter could reflect blood vessel diameters accurately. CDE revealed the perfusion of HV and IVC and the location of blood flow more ideally.The result of 70 BCS patients examined with Method A and Method B showed that: (1) There were 78 pathological hepatic veins, 70 of which were detected by Method A while 60 of which were detected by Method B. So the difference is of great significance and P is less than 0.05.(2) There were 57 abnormal IVC patients , 52 of which were detected by Method A while 43 of which were detected by Method B. Thus thedifference is of great significance and is P less than 0.05. In addition, 3 patients with thrombosis were detected by Method A while none by Method B. (3) There were 60 patients having formed hepatic collateral branch vessels and all of them were detected by both of the two methods, so there were no differences. In addition, 8 patients with small thrombus in collateral branch vessels were detected by Method A while none by Method B, so the difference in the detectable rate between the two methods is of great significance.Conclusion: Our results suggest that gray-scale ultrasound can only provide information about inner diameter, lumen and occlusion of HV and IVC. Color and spectral Doppler ultrasound can display the color and direction of blood flow in the HV, communicating branches and IVC and changes of blood velocity curve in the HV and IVC. Especially the combination of CDFI and CDE enhances the detectable rate of abnormal vessels evidently and may play an important role in the diagnosis of Budd-chiari syndrome.
