Diabetes which happened by genetic factors,immune disorders,microbial infections and toxins,free radical toxins,mental factors and so on , that act on the body result in islet dysfunction and insulin resistance (Insulin Resistance, IR), etc, triggered sugar,protein,fat,water and electrolyte series metabolic disorder syndrome.As social and economic development, people's dietary energy increased, but decreased physical activity,fast pace of life and other life style changes lead to the diabetes incidence increased rapidly worldwide, China is one of the highest prevalence of diabetes,with diabetes diseased prolonged,its nephropathy incidence gradually increased.Diabetic nephropathy is one of the most common complication of diabetes,it is also the leading cause of the death of diabetic patients.Renal biopsy in patients with diabetic nephropathy existed histological changes in the early,while it is usually no clinical symptoms,renal function and urine laboratory examination showed no obvious abnormalities,so early diagnosis is often overlooked,and end-stage renal disease occurred in a short time,so the quality of life of diabetic patients were affected. The diagnosis of diabetic nephropathy based on detection of urine protein,urinary albumin,plasmaurea,creatinine and other indicators as the primary diagnosis,these indexes are difficult to find early changes in kidney. Color doppler ultrasonography is easy to operate, for the patient ,it is no pain, no radiation, intuitive, repeatable, etc. It can detect intrarenal vascular flow para meters, reflect diabetic nephropathy damage, plays an important role on early diagnosis diabetic nephropathy. In this paper, combined with CDFI in patients with diabetic nephropathy and the CDE examination, Understanding about renal size,cortical thickness,renal artery blood flow and the parameters flow of vessel branches,etc, Further explore the early diagnosis of diabetic nephropathy in order to guide the prevention and treatment.Objective: To explore the value of combining Color Doppler Flow Imaging with Color Doppler Energy in diagnosis of diabetic nephropathy.Materials and methods: Collection August 2009-February 2010 in our hospital (male 26 cases, female 11) and clearly diagnosed diabetic nephropathy patients 37 cases (normal renal function 25 cases and renal change 12 cases) , normal group 20 cases excluded other factors such as primary nephropathy and hypertension. Using Acuson computer color doppler imaging diagnostic, the probe frequency is 3.5MHZ, Ultrasound combining color doppler technical observing the kidneys size, shape, cortex thickness, normal group20cases. CDFI take the kidneys oblique coronal section and display renal hemodynamics,using the same method CDE observation kidneys branching vessels flow,then use CDFI and CDE respectively to measure the value of renal aorta,interlobular artery and arcuate artery various flow parameters,including systolic peak velocity (Vs), diastolic minimum flow velocity (Vd) and RI.Results:1,Compared the kidney shape, size, cortical thickness between the control group and the DN group.The control group20cases, showed the outline of the kidney and kidney size low voice echoed,the massive cortical thickness was normal,rich in the bloodstream;25 cases which DN group in renal insufficiency, showed the outline of the kidney clarity, the size was normal,only the internal size slightly increased, and the massive cortical thickness was normal; 12 cases which DN group kidney function of a generation,showed renal cortex squeezd,echo strengthen internal.2,The Comparison of renal blood flow distribution between CDFI and CDE in 37 cases of patients with DN.Among DN patients CDFI showed 14 cases of typeâ… ,â…¡in 22 cases,â…¢8 cases,â…£type 30 cases; CDE showed 17 cases of typeâ… ,â…¡in 31 cases,â…¢type 19 cases,â…£in 7 patients, contrast of two groups showed: for the low flow vascular flow signals CDE was sensitiver than CDFI. The results shown in Table 3 and Figure 1.3,The Comparison of renal blood flow parameters between CDFI and CDE in 37 cases of patients with DN.Statistical analysis:There is a difference in the interlobar artery peak systolic velocity and diastolic minimum velocity between CDFI and CDE (0.01

0.05), the CDE in the display of parameter in the low flow, branching blood vessels is better than CDFI, see Table 2 and Figure 3-4.Conclusion:1,Further confirmed the value of CDFI in the diagnosis of DN.2,CDE is sensitive than CDFI in the peripheral blood and low-speed signals, particularly in the measurement of the arcuate artery and branches .3,Combining Color Doppler Flow Imaging with Color Doppler Energy andcombining with clinical have some value to improve the diagonsis rate ,prevention and treatment of DN.
