Objective In this study , we combine 131 cases of lumbar intervertebral disc herniation patients to study the clinical character of lumbar intervertebral disc herniation patient and the pathological character of the degenerated lumbar intervertebral disc, to study the principal etiopathogenesis of lumbar intervertebral disc herniation ,and to provide theoretical base for the prevention and treatment of lumbar intervertebral disc herniation.Method (l)To analyze the clinical data of 131 cases of lumbar intervertebral disc herniation patients who were treated in the orthopaedics department of Qilu Hospital from the year of 2000 to 2004 by statistical analysis from sex, age , occupation , position of herniation , BMI etc. Age distributing: 25 cases between 16 to 30 years old, 40 cases between 31 to 40 years old, 45 cases between 41 to 50 years old, 21 cases over 51 years old; Occupation distributing: 32 cases of heavy manual worker , 17 cases of moderate manual worker , 17 cases of comfortable manual worker, 22 cases of brainworker; Extruded position distributing: 76 cases in the position of lumbar 4 to lumbar 5, 53 cases in the position of lumbar 5 to sacrum 1, 5 cases in the position of lumbar 4 to lumbar 5 to sacrum 1 ; Body weight distributing: 29 cases of normal , 23 cases of overweight , 25 cases of fatness.(2)Observing the hyperplastic degree of the cartilage like cell and the fibrosis degree of the nucleus pulposus.Result (1) The invasion age of lumbar intervertebral disc herniation is often between 31 to 50 years old, especially between 41 to 50 years old. (2) Heavy manual worker andbrainworker are absolute majority . (3) The position of lumbar intervertebral disc herniation is mostly in lumbar 4 to lumbar 5? secondly in lumbar 5 to sacrum 1 . (4) The invasion of lumbar intervertebral disc herniation is frequently in overweight and fat person .(5) 41 percent of the patients have damage history , 53 percent of the patients have not damage history . (6) With the rise of age , especialy after 30 years old ,the cartilage cells of the extruded nucleus pulposus will be hyperplastic and the fibrosis of the tissue will aggravate .The group age below 20 has significant difference with other four groups (P<0.01), the group age between 21 to 30 has significant difference with group C, D, and E (P<0.05),and there is no significant difference between group C, D, and E.Conclusion The invasion of lumbar intervertebral disc herniation is intimate with age , occupation , position of herniation and BMI. The lumbar intervertebral disc cataplasia of age is natural and irresistible,but some anthropogenic factors accelerating the cataplasia can be prevented, and these factors are important conditions to prevent or relieve the invasion of lumbar intervertebral disc herniation.
