Objective: To investigate the variations in serum interleukin-2 (IL-2), interleukin-10(IL-10), tumor necrosis factor alpha(TNF- α ), INF- γ in monkey of resuscitation after selective cerebral ultra-deep hypothermia and blood flow occlusion. By means comprehention effects of monkey's immunological function resuscitation after cerebral ischemia selective cerebral ultra-deep hypothermiac blood flow occlusion.Method: Ten 4-10 year-aged maca mulattas were divided into three groups,four-vessel occlusion group, normothermia perfusion group and two-vessel occlusion group. Exposed bilateral exteral jugular veins (FJVs), bilateral exteral carotid artery(ECAs), bilateral internal carotid artery(ICAs) and bilateral internal jugular vein(IJVs), four-vessel occlusion group exposed bivertebral artery (VA).Right IJV intubated To monitoring central venous pressure(CVP).A catheter was inserted into right ICA to infuse cold Ringer's solution, two catheters were distally and proximally inserted into IJV to extract the hypo -thermic diluted blood for ultrafiltration and then perfuse the warm blood into the right venae cava inferior after rewarming. Total body was heparinizated before reduce the temperature. Both ICA were clamped 10 minutes(four-vessel occlusion group were clamped VA ). Then the both EJVs and the left IJV were clamped about 60 minutes. 4℃ cold solution was immediately perfused intothe right ICA to induce the brain cooling less 16°C.After 60 minutes Perfusion was stoped and blood vessels were recovered blood stream to cause brain physio-rewarming. Preoperative and postoperative examinated MRI. Operation changes of hemodynamics were measured;the dysfunctional scales of neurology were done every week.Venous blood was collected and centrifugated at 1500g/min to get blood serum conservated in -70°C refrigeration before perfusion and postoperative 24 hours, 72 hours. All the levels of these cytokines were measured by enzyme-linked immunosorbent assay(ELISA).Results: In all 10 monkeys, four of two-vessel occulation were succeeded in being build up the models . the hemodynamical parameters were steady during the operation, and all of them lived up for ever. MRI were normal after operation and the function of neurological deficient scale was normal. The levels of the four cytokines were significantly increased after perfusion(p < 0. 05) ,descended to pre-operation levels after 24 huors, the levels 72 hours were not significantly difference than pre-operation levels. All of the monkys of normothermia perfusion group and four-vessel occlusion group were not resuscitation after perfusion and died. In four-vessel occlusion group, the level of TNF-a , IL-lOafter perfusion was more significantly increased than two-vessel occulation group(p < 0. 05).Conclusion: It is safety that monkey resuscitate from selective cerebral ultra-deep hypothermia and blood flow occlusion of bilateral internal carotid for 60 min and it is normal to the hemodynamical parameters, brain and nervous function. The levels of proinflammatory and anti-inflammatory cytokines were significantly increased after monkey of resuscitation after selective cerebral ultra-deep hypothermia and blood flow occlusion. Because of pro-inflammatory and anti-inflammatory cytokines were effected each other, so two types of cytokines increasedat the same time to can cytokine network keeping dynamic equilibrium. That can advantageous reconstrct immunological function. So normal body may be no harm for cytokines transient significantly increase. Just is responses for body self-protection. But severe patienes maybe infected with pro -inflammatory and anti-inflammatory cytokines opportunit disequilibrium So it is necessary to consider variations of cytokines if selective cerebral ultra-deep hypothermia and blood flow occlusion was used in clinical.
