| Objective To explore the relationship between the pre-definitive treatment time and the prognosis of patients with severe polytrauma and to verify the improved result of severe polytrauma by shortening the pre-definitive treatment time as well as to provide the foundation to take measures in clinic for shortening the pre-definitive treatment time.Methods The clinical data of 188 cases with severe polytrauma admitted into the emergency center of a rank 3 hospital from March 2003 to April 2005 were recorded with indicators such as the pre-hospital time(T1), the detained time in the emergency department(T2), the pre-definitive treatment time(Tt=T1+T2), ISS score and so on. Then these cases were divided into six different subgroups as follows: 'group pre-hospital time ≤12h', 'group pre-hospital time>12h', 'group emergency operation', 'group EICU with non-emergency operation', 'group organ bleedingoperation' and 'group non-organ bleeding operation'. The data of these groups were analyzed by the regression statistical method in SPSS 13.0 to research if the pre-hospital time(Ti), the detained time in the emergency department(T2) and the pre-definitive treatment time(Tt=Ti+T2) was correlated with the prognosis and to compare the outcome of different time group after getting the turn-point time by Chi-square test.Results The detained time in the emergency department in three groups—'the total group', 'group pre-hospital time^l2h' and 'group EICU' was correlated with the prognosis of patients. The turn-point segment was '6-8h' in the 'total group'. Based on the turn-point time-'6h', the total group was divided into 'group T2=^6h' and 'group T2>6h'. The prognosis in the 'group T2^6h' was better than that of 'group T2>6h'(P<0.05).Conclusion The detained time in the emergency department is correlated with the prognosis of patients with severe polytrauma. Shortening the detained time in the emergency department and providing the definitive treatment early can improve the result of patients with severe polytrauma including those patients not done the emergency operation. The prognosis of those patients with severe polytrauma being received the definitive treatment within 6 hours will be better than that of beyond 6 hours. |
