American National Institutes of Health (NIH) has affirmed in due form that liver transplantation offers a lifesaving treatment for individuals with terminal disease since 1983. Since 1995, liver transplantation has been developing rapidly in China. In the years of 2003 and 2004, 3200 patients got the operation. Tianjin first central hospital has carried nearly 1000 patients with terminal disease. We found that patients in preoperative period present different degree depression according to investigation to 300 patients. Psychics research has approved that immunity system function related with mood nearly. The number of white blood cell in periphery veins rises when depressed, but at the same time the competence of nature killer cell falls. Research showed that surgery technology had matured, the prognosis of the patients lied on the immunity tolerance. The white blood cells and natural killer cells possess the function with immunity modulation. Then, the task how the clinical nurse tell the patients failure, goodness, convenient and suffering is so important.Objective: The study aimed to draw and put in practice a normal, practical and systemic intervention program for adult pre-transplantation patients with depress. The program can help the patients break away from depress gradually, adapt the role and bring up well healthy behavior.Methods: We screen out 70 patients in the waiting period between 2005 August and 2006 January. They were divided into trained group and control group randomly. 35 subjects received hierarchical system support which including 2-week healtheducation(2 times a week, 1 hour per time), 1-week panel discussion(30 minutes per time) and individual guidance(2 times a week)and another 35 comparable patients received intervention in common use. The total time is 6-8 weeks. The effect was assessed using SDS, Barthel, and the number of white blood & the natural killer cell. Statistic methods: all the data were copied into SAS software. The quantitative data were analyzed by t-test, analysis of variance and the qualitative data were analyzed by chi-square test. The two-tailed probability is 0.05.Results: After intervention, SDS exponent of trained group is lower than control group and Barthel of trained group is higher than control group(p<0.05). The number of white blood cell of trained group is higher than control group, t is 3.37, p (0.05. We analyzed the data of trained group and found that age, culture and economic were associated with depress.Conclusions:Hierarchical support can lighten the patients' depression gradually. In improving ADL degree and physiological guideline, it is better than the present support.
