Abstract/Summary:
Colorectal carcinoma (CRC) is a common gastrointestinal malignancy in human. Hepatic metastasis is a major death reason in the patients with CRC. The angiogenesis of tumor supplying vessels and the alteration of intra or extra hepatic blood flow may be found in the liver metastasis progress in the patients with CRC. Platelet derived-endothelial cell growth factor (PD-ECGF), i.e. thymidine phosphorylase (TP), is an angiogenic factor with angiopoietic ability. In order to detect the earlier hepatic metastasis, especially the occult cases, in the patients with CRC, study the alterative rule in PD-ECGF expression and the angiogenesis of this factor in CRC, 30 patients with CRC were preoperatively examined to determine Doppler Perfusion Index (i.e. DPI, hepatic arterial- total liver blood supply ratio), meanwhile 30 healthy persons were examined as the normal control. The mRNA and protein expression levels of PD-ECGF in 30 samples of CRC and normal distal tissues collected from the same patients group were assayed by real-time PCR and immunohistochemical methods. The results show that:1. The DPI of CRC patients with hepatic metastasis is significantly higher than that in the normal control with appearing the index accuracy of 100%.2. PD-ECGF mRNA and protein expression levels in CRC are significantly higher than in normal control.3. The higher expression of PD-ECGF is located inside of the interstitial cells in CRC.4. There are correlations between PD-ECGF expression level and CRC invasion, lymph node metastasis as well as Dukes' stage respectively, but no correlation between PD-ECGF expression and pathological typing orgrading is found.5. The mRNA and protein expressions of PD-ECGF in CRC appear statistical unanimity.6. There are positive correlations between PD-ECGF expressions and Doppler perfusion index separately.Conclusions:1. DPI may be a monitoring factor to scan hepatic metastasis in patients with CRC and serve as a practical measurement in postoperative following-up.2. PD-ECGF expression is significantly higher in CRC. In vitro PD-ECGF has the effect to promote endothelial cell growth and chemical chemotaxis, in vivo it has angiopoietic activity. It is very critical that PD-ECGF can critical lead to the change of intra and extra hepatic blood supply in liver metastasis of patients with CRC.3. PD-ECGF may be used to evaluate the prognosis of patients with CRC.4. According to the findings of molecular biology companying with the examination of Doppler perfusion index, some potential occult hepatic metastasis might be clinically detected in the advanced scanning regime for earlier diagnosis of liver metastasis in patients with CRC.
