A Study Of Ascitic Fluids LPA Levels And Expression Of Membrane Receptors(EdgRs) And Nuclear Receptor (PPAR-γ) In Ovarian Carcinoma

Objective To determine whether elevated levels of Lysophosphatidic Acid(LPA) are present in ascites from patients with ovarian epithelial carcinoma, and to evaluate whether an elevated LPA ascites level not only has a significant value in diagnosis of these diseases.but is helpful in determination the nature of ascites.At the sane time.the expression of endothelial differentiation gene-encoded G protein-coupled receptors (EdgRs) and nuclear receptor of peroxisome proliferators activated receptors gamma(PPAR-Y) were detected in ovarian tumor tissue and compared with ascites LPA levels to determine whether they are significantly correlated. Methods LPA were separated with lipid extraction and chromatography from 45 ascitic specimens of ovarian epithelial carcinoma,8 of ovarian non-epithelial carcinoma,5 of ovarian borderline carcinoma, 15 of ovarian epithelial benign tumors, 12 of ovarian endometriotic cysts and 4 of simple hepatic sclerosis patients, then quantified with chromatometer. The ascites levels of LPA were compared with serum CA125 levels of ovarian malignant tumors and nonmalignant diseases,to determine their value of diagnosis and wheather or not LPA is useful to determine the truth of ascites. The immunohistochemistry SP method was used to detect the protein expression of Edg2 ,Edg4 and PPARγ of LPA receptors in ovary of 24 patients with ovarian malignant tumors and 18 with ovarian nonmalignant diseases. Results Patients in the ovarian epithelial carcinoma group(5.51±1.75μmmol/L), ovarian non-epithelial carcinoma(5.12±1.11lμmmol/L)and ovarian borderline carcinoma(4.19±0.71μmmol/)had significantly higher ascites LPA levels compared with the ovarian epithelial benign tumors(2.11±0.44μnmol/L), ovarian endometriotic cysts(2.14±0.67μmmol/L)and simple hepatic sclerosis patients(1.75±0.85μmmol/L). There were no significant difference of ascites LPA levels between ovarian borderline carcinoma and ovarian carcinoma.,like ovarian epithelial benign diseases and simple hepatic sclerosis patients. Ascites LPA levels of ovarian epithelial carcinoma had norelationship with pathologic types and pathologic stages,but were relative with clinical stages and lymphaticmetastasis.And ascites LPA levels was more specific to ovarian epithelial carcinoma than serum CA125 levels in our study, but less sensitive than serum CA125 levels,but there was no significant value in statistics. The expression of Edg4 and PPAR-y in the ovarian epithelial carcinoma group was higher than that in ovarian nonmalignant diseases, while there was no distinctive expression of Edg2 between ovarian malignant tumors and nonmalignant diseases,and had no relationship with pathologic types, pathologic stages, clinical stages and lymphaticmetastasis. The expression of Edg4 was relative with clinical stages, pathologic stages and lymphaticmetastasis. The expression of PPAR-y was relative with clinical stages, and lymphaticmetastasis.The overexpression of Edg4 and PPAR-y in the ovarian epithelial carcinoma was positively correlated with the higher levels of ascites LPA, while there was no correlation between LPA and Edg2. The expression of Edg4 was positively correlated with PPAR-y,while there were no correlation between Edg2 and Edg4,PPAR-y.Conclusion 1. Ascites LPA may be the tumor marker of ovarian carcinoma,be helpful in the diagnosis and different diagnosis of ovarian carcinoma ,and in judging the truth of ascites.2.LPA was not only active in the genesis.but in infiltration and metastasis,and be helpful in the early diagnosis.3. Ascites LPA was a better marker campareing with serum CA125.4. Edg4,PPAR-y join in the growth, infiltration and metastasis of ovarian carcinoma,while Edg2 took part in the negtive regulation. 5. LPA was positively correlated with, Edg4 and PPAR-y,but had no correlation with Edg2.
