The Quantificational Study Of Spiral CT And The Depth Of Laryngeal Carcinoma In Pathology

Object : Laryngeal carcinoma is familiar epidemic cancer .Theincidence of the disease is 4.6/10000 in American, and 4.3/10000 inChinese. The invasion of Laryngeal carcinoma have relationshipwith blood supply and degree of carcinoma division . Supraglottiscancer invade more often then glottis cancer because blood supply.Nowaday , The therapy means of laryngeal cancer is operating.In 1874 doctor Billroth finished the first operating of larynx .And in1952 doctor Alonso and Jackson invented the laryngeal operatingconserving function of larynx. Since then more and more operatingconserving function of larynx were invented. The result is better thenbefore.All these achievement is based on accurate diagnosis oflaryngeal cancer and deciding to the border of laryngeal carcinoma.New studying find that quadrangular membrane ,conus elastius andthyroid cartilage have important function to prevent carcinoma cellinvading. These important structure limit cancer invading .It becomereasonable to conserve part of laryngeal function. So it become veryimportant to decide the border of laryngeal carcinoma accurately.All the time the way we used to classify stages provide a classicmeans to cure disease. But it can not tell doctor the accurate depth oflaryngeal carcinoma. So I try to fine a way to measure the depth oflaryngeal carcinoma with CT.Means: We select 28 in-patents of laryngeal cancer fromJanuary to September 2005 in ji lin university the second hospital,(17 supraglottis,11glottis cancers).Seven of them are cartilageinvaded .Everyone have been scanned from epiglottis toinfraglottis before operating with the parameter: windows wide400 , windows level 60 , thick slice 1mm , slice intergap1mm.measure the depth of laryngeal carcinoma with CT andimmunohistochamical means. Contrast and study the results.We found that there was some change in volume after thesample surgical removed be fixed with 10% formalin ,consideringthat the fresh sample might lose some water . The relationship ofchange could be got by a beforehand trial. The tissues length afterfixed was 0.857 times of the fresh tissues .Results : SCT can reflect the depth of laryngeal carcinomainvasion accurately. The average invade depth gap of CT andpathology is 1.9810土0.68529,and ninety percent in the range from1.67-2.29mm.
