Objective Our study detect the dynamical variation of serumcardiac troponin I in infants undergoing open-heart surgury withcardiopulmonary bypass to observe the cardioprotective effects ofproprofol and isoflurane in infants during cardiopulmonay bypass. Method Twenty-seven cases of infants with congenital heartdisease (arterial septal defect and/or ventricular septal defect) adoptedby exreacorporeal circulation under the intracardiac operation wererandomly devided into three groups according to anesthetic: isoflurane(9 cases);proprofol (10 cases) and fentanyl (8 cases as control),respectively.During the operation, in fentanyl group use fentanyl;inproprofol group (during exreacorporeal circulation) adding proprofol5mg/kg/h;in isoflurane group adding isoflurane (assuring to beaspirated 1.0%-1.5% isoflurane more than 30 minutes beforeexreacorporeal circulation). Drawing the blood of the center venous inorder to determine the level of the blood serum cTnI, duringpost-anesthesia and before the operation (T0),after ascending aortaopened 30 minutes (T1),post-operation 6 hours (T2),post-operation24 hours (T3), respectively. result: the level of cTnI in the three groups are increased indifferent degrees after aorta opened 30 minutes later;the level of cTnIreach the peak from aorta opened 30 minutes later to after operation 6hours;the level of cTnI after operation 24 hours decends slightly, butstill higher than normal;in the two observing groups, the level of cTnI(T1,T2,T3) are lower than those of the control group (P<0.05),there are not significant difference between the two observing groups(T1,T2,T3).Conclusion: 1,the level of cTnI reach the peak from aortaopened 30 minutes later to after operation 6 hours;the level of cTnIafter operation 24 hours decends slightly, but still higher than normal;2,proprofol and isoflurane play an important role in protectingmyocardium during CPB under the intracardiac operation under directvision. 3,the cardioprotective effects of proprofol and isoflurane ininfants during cardiopulmonay bypass are same .
