Study Of Risperdal And Chlorpromazine's Effects On Gonadal Hormone Of Male First Onset Schizophrenia

Object: To study the difference of gonadal hormone base level between male first onset schizophrenia and healthy control, search atypical antipsychotics of risperdal and chlorpromazine's different effects on patients' gonadal hormone of E₂ P T LH FSH and PRL.Methods: 1. Subjects: 60 male first onset schizophrenias coincidence ICD-10 standard of schizophrenia or schizophrenia like psychosis. Score of PANSS exceed 60, primary school or high educational background, age 18-45 year old, antipsychotic and endocrine effective drug naive. 10 healthy male control. 2. Method: All patients were divided into two groups,patients in risperdal group were administrated with lmg risperdal in beginning and added to 2⁶mg in 10 days.Patients in chlorpromazine group were administrated with 50mg and added to 300-600mg in 10 days. If they suffered to extrapyramidal system drug side effects, antan would been added;If they suffered to sleep disorder clonazepam would been through oral or intramuscular injection. 3.Disease Severity degree evaluation before and after treatment. Evaluated Score of PANSS at baseline and 8 weeks after. Therapeutic effect were evaluated byPANSS scores decreasion (Scores after - Scores before). 4.Sample collection:after admission draw venous blood at triangle of elbow for gonadal hormone (including E₂ T P LH FSH PRL).6am of 2ⁿd, 14^th and 28^th morning draw venous blood for PRL. 6am of 56^th morning draw venous blood for gonadal hormone (including E₂, T P LH, FSH, PRL). Healthy control were draw venous blood at 6am for gonadal hormone (including E₂ T P, LH FSH PRL). Blood was added heparin to anticoagulation, centrifugalization at 1500r/min for 5 min, separated plasma and store at -20℃ 5.Ttesting methods: Electrochemiluminescence immunoassay, kit bought from Roche. Using double antibody sandwich method to test LH and FSH. Competition method to test PRL, P, T and E₂. 6. Statistical method :All clinical data and testing results was import computer for SPSS statistical analysis. After testing of normality, quantitate data by pairing data t test, independent sample t test, ANOVA and x² test. Mean was express as X±S, if P≥0.05 we thought as nonsignificance others as significance.Results: 1. There were no statistic difference of age, weight and height between patients and healthy controls (p>0.05) . There were no statistic difference of age, education background, marriage station, course disease type, family history, weight and height between risperdal and chlorpromazine groups (p>0.05) . 2 There were no statistic difference of PANSS score between two groups at baseline (p>0.05) . In other words, no disease severity difference between two groups. In each group there were statistic difference of PANSS scores before and after treatment(p<0.01). After treatment PANSS scores were statistic different between two groups (p>0.05) . There were no statistic difference of PANSS scores decrease between two groups(p>0.05). 3. But for LH(p<0.01), there were no statistic difference of E₂ , P L FSH and PRL between patients and healthy controls (p>0.05) . In both risperdal group and chlorpromazine group, there were no statistic difference of E₂, P T, LH and FSH level before and after treatment (p>0.05) . Comparing PRL level between two treatment groups in different time points, but for baseline (p>0.05 ) , 1 day, 2weeks, 4 weeks and 8 weeks after treatment, risperdal group had statistic higher PRL level than chlorpromazine group(P<0.05 or 0.01). 4. PRL level of risperdal group and chlorpromazine group in different time points: PRL rise 1 days after treatment, reachpeak at 4 weeks, begin to drop to 2 weeks level at 8 weeks.Conclusion: 1. Adult male first onset schizophrenias have same level of serum E2n P> T> $SH^ PRL as healthy controls but their LH are lower than healthy controls. 2. Before and after 2-6mg Risperdal or 300-600mg chlorpromazine treatment, adult male first onset schizophrenias' E2> P% T> Lf^ FSH level do not change notable. 3. Oral use Risperdal or chlorpromazine could raise PRL level just at lday , reach peak at 4 weeks, and drop at 8 weeks.Oral use risperdal or chlorpromazine could raise PRL of adult male first onset schizophrenias, but obviously high in risperdal treatment.
