Background: Idiopathic sudden sensorineural hearing loss (ISSHL) is common. Despite a high rate of automatic recovery, the patients who failed to recover would suffered from severely sensorineural hearing loss mostly. Today, the etiology of ISSHL is unknown, various treatments have been used to treat ISSHL, but most of the methods are still in dispute.Method: 129 cases of ISSHL treated were reviewed in this retrospective study. All of the patients were treated in the Sir Run Run Shao Hospital during 1998-2005. The patients were divided into 4 groups: 54 patients in group A treated with intravenous infusing 10ug prostaglandin El daily for 10 days;23 patients in group B treated with batroxobin, 10BU infused for the first day, and 5BU QID accumulated to 40BU altogether;39 patients in group C treated with both prostaglandin El and batroxobin, the Administration is the same of group A and B;13 patients in group D treated with 250ml low molecular dextran and 10mg dexamethasone intravenous infusion daily and 0.5mg vitamin B2 BID. The total hearing gain and low-frequency, language frequency, high-frequency hearing gain of the 4 groups were analyses.According to the patients' age <20, 20-29, 30-39, 40-49, 50-59 and over 60 years, they were divided into 6 groups, and the hearing gain of each group was compared.According to the history of hypertension or no, the patients were divided into 2 groups, the hearing gain of the group with hypertension and the group without hypertension was compared.According to the history of diabetes mellitus or no, the patients were divided into 2 groups, the hearing gain of the group with diabetes mellitus and the group withoutdiabetes mellitus was compared.According to the the existence of vertigo or no, the patients were divided into 2 groups, the hearing gain of the group with vertigo and the group without vertigo was compared.Results: The total hearing gain and low-frequency, language frequency, high-frequency hearing gain are analyses. The hearing gain of the 4 groups in total hearing gain and low-frequency, language frequency, high-frequency hearing gain all turned out to be no significant difference(P>0.05), but the hearing gain in high-frequency was significantly lower than that of low-frequency(P=0.036<0.05) and language frequency (P=0.014<0.05). When prognostic factors were considered, the interval between the onset of hearing loss and start of treatment(P=0.005<0.05) and the existence of vertigo(P=0.007<0.05) had significant correlation with the degree of hearing recovery. Other factors, such as age, sexuality, left/right ear involved, with/without hypertension of diabetes mellitus failed to proved correlated with prognosis(P>0.05).Conclusion: the usage of prostaglandin El and/or batroxobin was no beneficial to the hearing gain of ISSHL;the hearing loss of high-frequency got worse hearing gain than that of low-frequency and language frequency;Earlier treatment after the onset of ISSHL had better effect;the presence of vertigo indicated worse prognosis. Age, sexuality, left/right ear involved, hypertension, diabetes mellitus had no significant correlation with the prognosis...
