| Object: To observe the therapeutic effectiveness of alprostadil for idiopathic sudden sensorineural hearing loss(ISSNHL).To discuss and analyse the relevant factors related with prognosis of ISSNHL.Methods: Two hundred and forty-seven patients diagnosed as having ISSNHL in one ear,were assigned to either A group or B group according to different treatment.The A group with 88 cases were treated by methylprednisolone gination lidocaine batroxobin and so on.The B group with 159 cases were treated by alprostadil in addition to the same treatment as the other group.Patients in both groups with hypertension and diabetes were closely monitored for blood pressure and blood glucose.Some of patients in two groups recevied hyperbaric oxygen therapy.Hearing change was evaluated by comparing pre-treatment and post-treatment pure-tone average(PTA).Follow-up for 3 months.Results: 54 cases of the treatment group was clinically cured,24 cases significantly valid,42 cases generally valid,and 39 cases invalid;total effective rate(cured+ significantly valid + generally valid)was 75.47%;the control group,15 cases were cured,19 cases significantly valid,16 cases generally valid,and 39 cases invalid;total effective rate was 56.82%.by means of ?2 analysis,the differences between the two group were statistically significant(P = 0.002,?2 = 9.187).The analysis of prognostic factors showed that there was no significant correlation between the patient's gender,the side of the deaf ear,the presence of tinnitus and the prognosis(P> 0.05).The factor such as presence of dizziness or aural fullness,hearing curve type,whether to give hyperbaric oxygen therapy and whether with hypertension and(or)diabetes were found to have association with hearing recovery(P < 0.05).Patient's age,the degree of hearing loss and time interval between onsets of symptoms to initiation of treatment were significant factor related to prognosis(P < 0.05).There were linear relationships between the three groups and prognosis(P < 0.05).and there were negative correlation(rs < 0).Conclusion: Alprostadil treatment of ISSNHL can improve the curative effect.Normally,initial milder hearing loss and early medical intervene have positive impact on the prognosis.Patients with ascending pattern of hearing loss,without hypertension and diabetes,without concomitant dizziness and aural fullness restore auditory function better.Hyperbaric oxygen treatment can improve the therapeutic effect. |
