Objective:To study the sensitivity and specificity of Amsel criteria,BVBlue test and the Nugent method in diagnosis of Bacterial Vaginosis . Methods and Design:192 patients from 2005.4 to 2005.12 come from Tianjin General Hospital these patients had the symptoms including abnormal vaginal discharge,vaginal odor,vaginal irritation,vaginal itch.ROC CURVE , chi-square test were used for analysis.Result:1.Positive rate of Nugent method , Amsel criteria and BVBlue was 45.3%,27.1% and 17.7% respectively.They were tested by chi-square test.2.Combination of the BVBlue test positive result with VVC positive result was 5.7%. Combination Nugent method positive and VVC positive was 6.2%. Amsel method positive and WC positive was 5.2%.3.To evaluate sensitivity ,specificity,negative predictive value,positive predictive value of the Amsel criteria, the highest value of sensitivity was the presence of clue cells, 100%;the sensitivity value of vaginal pH>4.5,amine test and vaginal discharge was 98%, 83%, 54%. The specificity value of amine test, vaginal pH>4.5, vaginal discharge and clue cells was 97%,85%,69%,57%. PPV of amine test, vaginal pH>4.5, vaginal discharge and clue cells was 92%,71%,71% and 48%.NPV of the four criteria was positive amine test,94%,pH value was 99%. The lowest NPV value of the four criteria was vaginal discharge.4.The sensitivity , specificity,PPV and NPV of Nugent method was 96%, 100%, 100% and 96%. The sensitivity , specificity,PPV and NPV of Amsel criteria was 57%,99%,98% and 72%.The sensitivity , specificity,PPV and NPV of BVBlue method was 37%,100%,100% and 64%.5.Microscopic examination of the discharge:White cell exceed 50% per high-dry power indicate mixed infections. Most of discharge of B V was not mixed infection. Conclusion:1.Discharge examined in Nugent method, Amsel criteria and BVBlue positive rate is 45.3%,27.1%and 17.7% respectively.2.Combination of the evaluation test positive result with WC positive result is 5.2-6.2%.3. Vaginal pH>4.5 was the best index.Amine test was better.4. Nugent method was the best method based on sensitivity , specificity and ROC area.5.Microscopic examination of the discharge:White cell exceed 50% per high-dry power indicate mixed infections. Most of discharge of BV was not mixed infection.
