Objective: To investigate the difference of C-reactive protein and intercellular adhesion molecule-1 in subjects with impaired glucose regulation,and compare the relationships between CRP or ICAM-1 and insulin resistance,to study the relationship between macroangiopathy and impaired glucose regulation.Methods: 150 subjects participated in the study according to oral 75 glucose toletance. 30 subjects as healthy control group,30 cases with impaired fasting glucose (IFG) ,30 cases with impaired glucose tolerance (IGT),30 cases with IFG complicated IGT, and 30 cases with Type 2 diabetic mellitus (T2DM). They were measured in body weight,height, waist, hip circumference and blood pressure .To all participants the body mass index(BMI) and waist-to-hip ratio(WHR) were calculated.Overnight fasting blood samples were drawn for lipid,glucose,insulin,CRP,ICAM-l ,and other biochemical measurements.A standard 75g oral glucose tolerance test(OGTT) was performed.The CRP or ICAM-1 concentrations were detected with radioimmunoassay and enzyme linked immunosorbent assay(ELISA) respectively, IR index estimated used HOMA. The statistic analysis was performed by SPSS for 11.0.Means of continuous variances between-group comparisons were performed with Analysis of Variance ,Multi-variabale Logistic regression was used to analyze the affects of CRP and ICAM-1 to insulin resistance.A two-tailed P value<0.05 was taken as statistically significant.Results: (1)The BMI, WHR, Systolic Blood Pressure (SBP) and triglyceride (TG ) in T2DM were higher than in healthy control group;There were no significant differences in Total serum cholesterol, Low density lipoprotein cholesterol and High density lipoprotein cholesterol among five groups. (2) CRP,ICAM-l,Homa-IR were significantlyhigher(CRP,Homa-IR P<0.01;ICAM-1 P<0.05 )in T2DM group than in IGT and IFG complicated IGT group.(3) CRPJCAM-1 and Homa-IR were significantly higher( P <0.01)in T2DM group than in IFG and healthy control group.(4)CRPJCAM-1,Homa-IR were significantly higher ( P<0.01 ) in IGT,IFG complicated IGT group than in IFG and healthy control group. (5)After controlling age %weight etc,stepwise multiple linear regression analysis suggested BMI,CRP and ICAM-1 were important risk factors of Homa-IR.Clnclusion:(l) There were significant insulin resistance in IFG ,IGT,IFG complicated IGT and T2DM groups than in controls. CRF\ BMI and ICAM-1 were risk factors of Homa-IR;(2)With the worsening of glucose tolerence, CRP ,ICAM-1 concentrations were gradually increased;(3) There was not only endothelial dysfunction in impaired glucose regulation group,but also some macroangiopathy (acute phase response protein, CRP);Postgradute: SHANG Yong-fang (Endocrinology) Directed by: YUAN Ying...
