Objective To study correlation between the adiponectin-leptin ratio and parameters of insulin resistance in patients with type 2 diabetesMethods (1) We enrolled 160 patients with type 2 diabetes (78 men and 82 women). (2)We studied the correlation between these parameters of insulin resistance ( Body mass index BMI,waist hip ratio WHR,triglycerides TG,HDL cholesterol HDL)and adiponectin,leptin, homeostasis model assessment (HOMA-R),adiponectin-leptin(A/L)ratio (3)The patients were divided into 3 groups classified by their fasting plasma glucose level(below 7.8 mmol/L,group A;between 7.8 and 9.4 mmol/L,group B;and 9.4 mmol/L or above,group C),we investigated the correlations between these parameters and A/L ratio or HOMA-R,and the correlation between A/L ratio and HOMA-R in each group.Results To study correlations between these laboratory parameters and adiponectin, leptin, HOMA-R,or A/L ratio. Adiponectin levels correlated significantly with WHR , TG . Leptin levels correlated significantly with BMI.HOMA-R correlated significantly with BMI.On the other hand, A/L ratio significantly correlated with BMI, WHR, TG.Next, the patients were divided into 3 groups classified by their fasting plasma glucose (FPG) level, and the correlations between the parameters and A/L ratio or HOMA-R, and the correlation between A/L ratio and HOMA-R were investigated in each group. In group A, A/L ratio correlated significantly with BMI, WHR, and TG. In group B, A/L ratio correlated significantly with BMI, HDL. In group C, A/L ratio correlated significantly with BMI, WHR, TG. On the other hand ,In group A, HOMA-R correlated significantly with HDL. In group B, HOMA-R correlated significantly with BMI. In group C, HOMA-R was not correlate significantly with these parameters of insulin resistance. On the same time, In group A, A/L ratio correlated significantly with HOMA-R. In group B, A/L ratio correlated significantly with HOMA-R. Significant correlations between the parameters and A/L ratio were tended to be observed as the FPG level rose, however, the significant correlations between the parameters and HOMA-R were no longer seen as FPG level elevated. In group A and B,A/L ratio significantly correlated with HOMA-R, as the FPG level rose. In group C, the significant correlations between the A/L ratio and HOMA-R were no longer seen.Discussion (1)The results suggested that the A/L ratio was effective in relevance as a parameter of insulin resistance to adiponectin,leptin or HOMA-R alone.(2) A more sensitive and reliable marker of insulin resistance than HOMA-R as the FPG level elevated, in patients with type 2 diabetes.
