[Objective] To study intercavernous sinuses and correlative structure by applied anatomy under operating microscopes and endoscopes so as to provide anatomatic data for transnasal surgery and endoscopic extended transnasal surgery, avoiding the injury of nerves and blood vessels and decreasing complication and improving approach; and to search anatomatic fixing mark of intercavernous sinuses and correlative structure by modeling transsphenoidal endoscopic sella surgery.[Methods] 20 adult cadavers(male 12,female 8) and 5 adult cadavers(male 4,female 1) were enrolled in the present study. Their intercavernous sinuses and correlative structure were dissected with clinical applied anatomy under microscope and endoscope, respectively. Some indexes (occurrence rate, position, morphous, diameter, distance to correlate structure) were observed for anterior intercavernous sinuses, inferior intercavernous sinuses, posterior intercavernous sinuses, basilar sinuses and dorsum sella sinuses. Meanwhile, the above indexes of correlative structure of intercavernous sinuses, such as sphenoid sinus, pituitary, intracavernous carotid, optic nerve, optic chiasm, arteria ethmoidalis were surveyed.Then the data were statistically analyzed by SAS package.[Results] ① Occurrence rate of anterior intercavernous sinuses, posterior intercavernous sinuses, inferior intercavernous sinuses, basilar sinuses and dorsum sella sinuses are 95%(19/20),75%(15/20), 10%(2/20), 100%(20/20) and 30%(6/20).Anteroposterior diameters of the sinuses were (2.15±0.90)mm, (5.14 ± 2.54)mm,(1.30 ± 0.40)mm, (2.26±0.91)mm, (2.01±0.80)mm, respectively. Supra-inferior diameter were (2.74±0.96) mm, (1.24±0.81) mm, (1.48±0.29) mm, (15.67±4.54) mm, (3.35±1.93)mm.②Intracavernous carotid of type " S " , of intermediate type and of linear type were respectively 37.5%(15/40),52.5% (21/40) ,10% (4/40) .③Sphenoid sinus of type II, III, IV, V were 25% (10/40),27.5% (11/40),27.5 % (11/40) ,20% (8/40) .And type I was not seen.
