Analysis The Surgery Treatment Of The Associated Acetabular Fracture

Objective To discuss the method for improving reduction quality of associated acetabular fractures in order to estabolish the treatment procedure.Methods From March 2000 to March 2005 , 43 cases with displaced acetabular fractures were managed surgically. According to Letournel classification method , there were 10 posterior wall ,3 posterior column , 4 anterior column , 5 transverse ,9 both column , 4 posterior column and posterior wall ,5 hemitransverse and posterior wall, 3 T shape fractceres. 26 case were included in associated fracture group. Simple classification concerning the anterior and posterior column characteristic of the acetabulum included anterior column fracture,posterior column fracture and double column fracture.The simple classification was the director of the incision.Six of associated fracture was perform surgery in Kocher-langenbeck incision,4 ilio-inguinal incision,3 extended ilio-femoris incision, 13 posterior-anterior incision. Reducement was performed on the column referring to its dislocation degree and bone block size,not sticking to do firstly on the anterior column. The anterior column was exposed in a triangle,otherwise posterior column a foursquare in surgery.We firstly identify all the edge of the triangle and foursquare,then lay reducing apparatus symmetrically as possible reckoning on triangle anterior column and foursquare posterior column and place the plate and implant the screws reckoning on the same. Results According to the reduction grade of Matta and clinical evaluation criteria of HHS (Harris hip score) , 22 cases obtained excellent reduction quality with a rate of84.6% (22/26) included 19 case of anatomical and 3 case of good.And the excellence rate by clinical evaluation criteria was 84.2%(16/ 19)to the anatomical group ,otherwise 25%(l/4) to the poor group.
