In plastic and reconstructive surgery, random skin flap is one of the most widely used flaps. The survival rate of the flap is the sign of the flap blood circulation. Survival feasibility of flap lies on blood circulation and the metabolizing of flap. In the initial stage of ultra long and broad skin flap formation, the blood supply of a flap mainly depends on its stem . The distal blood supply of the flap depends on not only the plasma circulation between the skin flap and recipient site but also the short-term maintenance of capillary reactive ectasis and congestion in the flap. In the following stage , the thrombosis occurs in the distal end of the flap because of the insufficient blood circulation and blood flow. As a result, the necrosis of the skin flap emerges because of the distal ischemia of flap.Some reseachers show that the ischemic/reperfusion tissue injury is a crucial cause of the flap necrosis, and a great mount of the oxygen free radical(OFR),inflammation factors and lipidperoxidation(LPO) produced in that process plays an important role in the mechanism of the flap necrosis.
