Object: To investigate the effects of Sodium Ferulate (SF) on survival of Dorsal random pattern flap in rats, and the possible mechanism.Method: A 8cm x2cm full thickness dorsal flap with the pedicle remaining attached at the posterior end was elevated in 36 healthy SD rats. They were randomized equally into 3 groups (SF_a group,SF_b group and the control group) of 12 rats each. 80mg/kg SF,40mg/kg SF,2.5ml N.S was seperately injected into the abdominal cavity of the 3 groups at the very time the flaps elevated. The same drug with the same dosage of each group were administrated at 12h, 24h, 36h, 48h after the operation. Information of the amount of food and drink, the spirits, the activitys and the state of the flaps were recorded at 24h, 4d, 7d after the operation. 6 rats of each group were sacrified at 24h after the operation. A round full thickness tissue sample about 1cm diametered at the center of each flap was taken for the examinations of SOD, NO, MDA level. 7d after the operation, the flap paddle survival of each group were measured by planimetry to get the survival area and proportion. Then the rats were sacrificed and the survival tissue of the flap was sectioned with haematoxylin-eosin staining for histological analysis to observe the amount of the inflammatory cell, the capillary, etc..Results: 1. 24h after the operation, in the flaps, the SOD content of SF groups were both higher than that of the control group and the SOD content of the SF_a group was higher than that of SF_b group, the MDA content of SF groups were both less than that of the contrl group and the MDA content of SF_a group was less than that Of the SF_b group, the NO content of SF_a group was higher than that of the other groups, which shows significant statistical difference (P<0.05).2. 7days postoperatively, light microscope observation showed that the flap tissue of SF_a group is less edema and with less inflammation cells than that of SF_b group, which is less edema and with less inflammation cells than that of the control group. The amount of micro capillary of SF_a group is more than that of SF_b group, which is more than that of the control group. The survival proportion in the flap of SF_a group is higher than that of SF_b group and the survival proportion in the flap of SF_b group is higher than that of the control group, which shows significant statistical difference (P<0.05).Conclusion: 1. The results support the notion that SF can increase the survival of Random pattern skin flap. With the dose increasing, the survival part of the flaps will improve.2. The possible mechanism that SF can improve the survival of flaps may be related to its effects of alleviating ischemical reperfusion injury and inducing angiogenesis of skin flaps.
