| Backgrounds The disequilibrium between pre-inflammatory and anti-inflammatory mediators is considered to be one of the main mechanisms of MODS leaded by SAP. Therefore, the key point for early treatment of SAP is how to eliminate pancreatitis-related mediators and to reach a perfect balance between pre-inflammatory and antiinfla- mmatory mediators. Now, activated NF-κB as an initial factor plays an important role in the pathogenesis of acute pancreatitis, activates a lot of inflammatory media,and induces cascading reaction of inflammation. TNF-a is a pivotal factor in pancreatitis pathogenesis,it cooperates with amylase and lipase to intensify the leisure in pancreatic tissue resulting bleeding and necrosis. The recently, it has been found that continuous renal replacement treatment could remove a lot of inflammatory mediators. Among them, high volume hemofiltration (HVHF)is more valid in renal replacement and inflammatory mediators removement. Objectives To study the effect of early short time high volume venovenous hemofiltration (ESHVVH) on plasma level of TNF-a , IL-6, IL-10, pancreas injury, and level of NF-κB in pancreas tissues; discuss the probably therapy mechanism of hemofiltration on SAP.
