Discussion On The Function Of Ovarian Reserve And The Rate Of Pregnancy After Ovarian Cystectosis And Ovarian Cystectomy Descend Or Not

Objectives:The ovarian cystectosis is common in gynecological tumor, it occupies 32% of the gynecogenitals tumor. It should take operation if the diameter of the cyst >5cm or it is a maglignant tumor. Well, if it will lead the function of ovarian reserve and the rate of pregnancy to descend after cystectosis and cystectomy using electrocoagulation ect. or not is still a subject in this field. We use the superovulation of ART to observe the influence which was inferred above.Materials and methods:1. Reviewing analysis of the patients who take IVF-ET in our center from 2001.1 to2006.12, divide several groups as follows:A1group: cystectomy of two laterals, 20 persons, 22 cyclesB1group: cystectomy of only one lateral, 36 persons, 43 cyclesC1group:cystectosis without achieving the standard to take operation and without undergong operation, 20 persons, 26 cycles2. Contrast group: After we chose the experiment group ,we select the first patient with the same age to compare as the contrast, concluding the tubo-fertilities, the male-fertilitis, but except the PCOS and the patients who stop the cycles with the low ovarian response.A2group: contrast with A1, 20 persons, 22 cyclesB2group: contrast with B1, 43 persons, 43 cyclesC2group: contrast with C1, 26 persons, 26 cycles3. Compare with A1 group and A2 group, B1group and B2 group , C1 group and C2 group about the number of antral follicles(NAF), the basalFSH(BF), the days of using GnRHa(DG), the beginning of Gn(BG), the E2 of the day using HCG(HCG E2), the number of follicles which≥14mm(≥14mmFc), the number of follicles which picked out(ORFc), the number of good embryos(GE), the number of freezing embryos(NFE), the pregnancy rate(PR), the number of transferring embryos(NTE), the diliver rate(DR). Compare the normal ovary to the operational ovary about B1 group, compare the normal ovary to the cystic ovary about C1 group, compare the operational ovary in B1 group to the cystic ovary in C1 group.Results:1. Compare with A1 group and A2 group, we find that NAF, BG, HCG E2,≥14mmFc, NF and NTE have the statistically significance(SS) but BF, DG and PR.2. Compare with B1 group and B2 group, we find that NAF, BG,≥14mmFc, ORFc have the SS but BF, DG, HCGE2, GE, NF, NET and PR.3. Compare with C1 group and C2 group, we can not find the SS between them but NAF.4. Compare the normal ovary to the operational ovary about B1 group: We find that≥14mmFc and ORFc have SS but NAF.5. Compare the normal ovary to the cystic ovary , we find that NAF,≥14mmFc and ORFc have SS.6. Compare the operational ovary in B1 group to the cystic ovary in C1 group, we can not find the SS between NAF,≥14mmFc and ORFc. Conclusions:Both the cystectosis and the cystectomy can influence the function of ovarian reserve but PR of ART. So we can take operation to cure the ovarian cystectosis if necessary, then we can ascertain the nature of the tumor. It should improve the technology of our doctors, in order to reduce the unnecessary electrocoagulation and suture, do our best to protect the function of the ovary.
