ObjectiveTo evaluate the effects of community-based case management for hypertensive patients for scientific evidence provision to standardize community-based case management and explore cost-effective intervention patterns for non-communicable chronic diseases(NCDs).MethodsTotal 700 hypertensive patients were selected by multi-staged randomized sampling method in Hsishu district, which were randomly divided into managed group (n=274) and control group (n=426). Patients in managed group and control group were followed up with systematic community-based case management and routine management, respectively.Results1 , Systematic community-based case management effectively promoted positive changes in related knowledge, attitude and behavior. Awareness rates and health related behavior rates in managed group were significantly higher than those in control group, which knew diagnosis criterion for hypertension, knew reasonable salt consumption amouts, knew auspice for stroke, and limited their salt consumption, joined in physical exercise and regularly took medicine after intervention. The pattern also improved self-management skills.2, Awareness rates and health related behavior rates in control group were also increased after intervention, and our study played an active role in acceptance and utilization willing to community health services.3 , Mean value of SBP and DBP declined after intervention both in managed group and control group. Mean SBP and DBP value reduced 7.6mmHg and 2.3mmHg respectively in managed group,while 6.3mmHg and 1.6mmHg in control group, respectively, there were significantly different between two groups (p<0.01) . Control rate increased from 28.3% to 53.9% in managed group and from 32.8% to 50.4% in control group, but there was no significantly different between two groups.ConclusionSystematic community-based case management for hypertensive patients was an effective pattern than routine management in community-based NCDs management.
