Objective High-dose intravenous immunoglobulin (HD-IVIG) has become the first choice in the therapy of patients with severe idiopathic thrombocytopenic purpura(ITP), especially in those whose platelets are very low and incline to get fatal bleeding in the skull or in the important organs , since HD-IVIG is very efficient to treat children with acute serve ITP. The HD-IVIG is too expensive to be used in a large range, so we tried to use middle dose IVIG(MD-IVIG) to treat the serve ITP patients whose platelets were very low or bleeding were serve. We observed the curative effect and the platelet parameter, wanted to find whether a lower dose of IVIG had the same effect as the high dose and the effect didn't decrease in the same time. We also examined the cytokines such as interleukin-2(IL-2),interferon-α (IFN-α),tumor necrosis factor-α(TNF-α) in children with ITP in order to confirm the changes and action of the cytokines in ITP patients. Methods 62 acute serve ITP patients were divided into three groups randomly. Group A was treated by the MD-IVIG, 0.5g/kg/d×2d. The total dose was 1g/kg, n=20. Group B was treated by the HD-IVIG, 0.4g/kg/d×5d. The total dose was 2g/kg, n=22. Dexamethasone was used in both groups. Group C was treated by dexamethasone only, n=20. The...
