Object: Laparoscopic rectal excision is a more technically demanding procedure. Open resection for rectal cancer should not only follow similar oncological principle,but the principle of total mesorectal excision as well. The trauma of surgery modifies major components of host defense and affects tumor resistance mechanisms. Consequences of surgery-related inflammation affect the development of antigen-special cells and alter the balance of cell-growth regulatory and angiogenesis-stimulatory factors, and may result in the inhibition of tumor resistance. Laparoscopy and laparotomy exert different effects on the peritoneal immune system. It is controversial that carbon dioxide pneumoperitoneum leads to increased tumor metastases. As laparoscopic sugery continues to be used in rectal cancer,it is very important to evaluate laparoscopic rectal surgery and host immune responses to surgery. The aim of this study was to compare the alterations of peritoneal and systemic immune response and the perioperative outcomes after laparoscopic and open surgey for rectal cancer.Methods:From Apr.2005 to Feb.2006,40 patients with rectal cancer underwent rectal resection.20 patients underwent laparoscopic and 20 open rectal excision.WBC,CRP,IL-6,CD4,CD8,CD4/CD8,NK cells and lymphocyte in peripheral blood were counted and compared on the 1st day before operation, 1st and 3rd day after operation.
