IntroductionPSA is a glycoprotein which is produced by the epithelial cell of prostate tissue, its organ-specificity has revolutionized not only prostate cancer screening detection, but also the surveillance of patients with prostate cancer after therapy. BPH is very common among aging men. In patients with BPH, The development of acute urinary retention as well as BPH-related interventions become more likely as prostate size increases. Recent data indicate that prostate volume is positively related to PSA. While previous studies have examined only patients with clinically diagnosed BPH who were presented with lower urinary tract symptoms (LUTS), but BPH was not considered to be the only etiological factor in men referred as a result of LUTS. Our study analyzed the serum PSA and PV in patients with histologically proven BPH, regardless of their clinical presentation, to further validate the relationship betweenPSA and PV and evaluate the role of PSA in the managing the patient with BPH.Materials and methodsFrom June 2003 to December 2005, 152 outpatients were confirmed to have BPH as the only histological diagnosis after evaluation with serum PSA, trans-rectal ultrasound (TRUS) biopsy, and prostate volume measurement. PSA and prostate volume were statistically analyzed by age-stratified cohorts, including multiple regression analysis and assessment of correlation using the Pearson correlation coefficient (r).Resultsthe age had a slight correlation with PSA(r=0.19, P<0.05), but no correlation with PV(P>0.05). The correlation of PSA and prostate volume was determined to be statistically significant (P<0.05) in each cohort, and the correlation coefficients in the cohort of 51 to 60 years, 61 to 70 years, 71 to 80 years are 0.63, 0.36, 0.36 respectively. The overall correlation coefficient was 0.38, but the difference between each successive age cohort were statistically insignificant for both mean PSA (P<0.05) and mean prostate volume(P<0.05). The mean value of PSA density was 0.21±0.15ng/(ml×cm3), and the PSA density had no correlation with age(P>0.05).ConclusionProstate volume is positively related to PSA in patients with biopsy-proven BPH. Serum PSA may predict prostate volume in a certain degree, therefore, it can be used as an aid to evaluate the patients with BPH in clinical practice.
