OBJECTIVE: To analysis the 42 patients whose age are 40 years old or elder with gestational trophoblastic disease(GTD) , and to study the clinical value of the methods of treatment of GTD.METHODS: To analyse theclinical and pathological data of fourty-two patients with gestational trophoblastic disease treated at Women's Hospital of Zhejiang University School of Medicine, China.from March 1990 to December 2004.RESULTS: The patient's age ranged from 40 to 53 years (mean 46.195 years old). Within the 42 patients, 8 were diagnosed with hydatidiform mole(HM), 28 were Invasive Mole (IM), and 6 were Chorionic Carcinoma( CC). All of the patients with gestational trophoblastic tumor (GTT) were divided into two groups (According to the FIGO staging and prognosis scoring system, high risk and low risk). Of all hydatidiform mole patients, 3 patients were treated with evacuation. 2 patients were selected for hysterectomy without following chemotherapy. Other 3 patients (1 with high risk factors) were both selected for hysterectomy and chemotherapy. Of gestational trophoblastic tumor patients, all of the GTT patients (lOwith high risk and 24 with low risk) were selected for hysterectomy and postoperative chemotherapy. 8 of the GTT patients with high risk and 23 with the low risk were cured, 1 patient with high risk (FIGO prognosis scoring is 17) died of the complication of the metastasis of tumor, and 1 patient with high risk and 1 with low risk refused to continue the chemotherapy gave and lost to contact on follow up. RESULTS: The surgery is important method of treatment for the women whose age are 40 years old or elder with gestational trophoblastic disease .
