Objective: To approach prevalence features of HCV genotypes in Changchun city and the significance of genotype in hepatitis C transmission,being chronic,severity of liver inflammation,serum load of HCVRNA and fatty denaturalization.Methods: HCVRNA and HCV genotypes of 82 patients with chronic hepatitis C,cirrhosis,hepatocarcinoma were assayed by fluorescent quantitation CR and polymerase chain reaction with type-specific primer. At the same time,the general data of the patients,the transmission approach,liver functions,and fatty denaturalization under B ultrasound were registered. We applicated SPSS for windows to statistic analysis,t test for measurement data and x2 test for numeration data.Results:1. 78 patients out of the total 82 were positive for HCV RNA. he ratios of HCV RNA gene type 1a,1b,2a,2b,3a were 5.1%(4/78),48.7%(38/78),33.3%(26/78),11.6%(9/78),1.3%(1/78). No mixed genotype was found. 2. The ratios of blood transmission in HCV RNA genotype 1a,1b,2a,2b,3a were 75.0%,71.1%,57.7%,44.4%,100% respectively,while that of scattered affections were 25.0%,28.9%,42.3%,55.6%,0.0%. There had no significant difference between these two groups.(p>0.05)3. The average load of HCV RNA in patients infected by genotype lb and 2a were 106.7±2.1copies/ml,105.4±1.7copies/ml respectively. The difference between the two groups was statistically significant (p<0.05). In the patients, the mean levels of ALT for genotype 1b and 2a were 64.8±13.7IU?L,63.4±23.1IU?L (P>0.05) respectively. The difference was not statistically significant,either. 4. 4 genotype 1a and 1 genotype 3a all were chronic hepatitis C . The distributions of chronic hepatitis C,cirrhosis and hepatocarcinoma in 38 genotype 1b were 25,8,5respectively, while that of 26genotype 2a were 20,4,2. There were 8 cirrhosis and 1 hepatocarcinoma in 9 genotype 2b. X2test,occurrence ratios of cirrhosis,hepatocarcinoma in genotype 1b was higher than that of genotype 2a significantly(p<0.05). 5. The occurrence ratios of fatty liver in genotype 1a,1b,2a,2b,3a were 25.0%,44.7%,19.2%,44.4%,0.0% with P<0.05. The difference was not statistically significant(P>0.05). But the difference between 1b and 2a was statistically significant (p<0.05).Conclusion: We found the prevalent HCV strains in Changchun city were genotype 1b and 2a; The difference was not statistically significant between HCV genotype and transmission route,ALT level; The virus level of genotype 1b was higher than that of genotype 2a., Perhaps that was a reason that genotype 1b had the worst response to interferon; The difference was statistically significant between HCV genotype and severity of chronic hepatitis C and occurrence of cirrhosis,hepatocarcinoma; Genotypic test can predict the development and prognosis of chronic hepatitis C; The genotype 1b had much influence on fat metabolism; The patients with genotype 1b were more prone to the occurrence of fatty liver.
