Objective To investigate the expression of proliferating cell antigen (Ki-67) , HPV16/E7and CD34 to study the relationship between HPV16/E7,tumor angiogenesis and cellular proliferation activity in laryngeal papilloma in adults.Methods 42 cases of laryngeal papilloma in adults from January 1998 to Dcember 2001 treated in ENT department of Jinan Center Hospital were retrospectively studied.All the patients were followed up more than 5 years. The age ranged from 24 to 66 years by means of 47. 15±2. 17 years. 28 vocal cord polyp and 28 laryngeal squamous cell carcinoma were served as control. In vocal cord polyp the age ranged from 22 to 54 years by means of 46.36±3.66 years. In laryngeal squamous cell carcinoma the age ranged from 40 to 77 years by means of 61.21±2.31 years. The exepression of HPV16/E7 was measured by polymerase chain reaction in laryngeal papilloma in adults. The expression of Ki-67 and CD34 was measure by immuneohistochemical SP in laryngeal papilloma in adults.Results The HPV16/E7 positive exepression could be detected in 4 cases among 28 cases of vocal cord polyp (14.3%), 28 out of 42 cases of laryngeal papilloma in adults(66.7%), 24 out of 28 cases of laryngeal squamous cell careinoma(85.7%). Difference were statistically significant(P<0. 01). In PVC, positive cells expression of Ki-67 only exist sporadically in stratum basale; In LP, the number of positive cells increases gradually, wh- ich exist sporadically in stratum basale, stratum spinosum or the whole epithelial layer, and in stratum basale the number of positive cells is greater than that in PVC; However, in LC, positive cells have such characteristics: nuclear pleomorphism, abundant nuclear chromatin, greater ratio of nucleus to plasm and abnormal nuclear mitosis. Additionally, the number of the positive cells which extend into middle layer of epithelium increases obviously. The labeling index of Ki-67 was 6.30 ± 2.03 in group PVC, 13.73 ± 3.42 in group LP and 25.55 ± 3.92 in group LC; Difference were statistically significant(P<0. 01);The wall of blood vessels in vocal cord polyp were completely and the blood vessels were not distributed densely. And in laryngeal papilloma . part wall of blood vessels were not completely and there were some blood vessels distribute densely, in laryngeal squamous cell carcinoma the wall of blood vessels is not completely. The number of blood vessels were significantly increased, and some endothelial cells distributed densely. The transversal profile of the interstitial angiogenesis in vocal cord polyp and laryngeal papilloma were regular, showed oval shape, round shape. And the transversal profile of the interstitial angiogenesis in laryngeal squamous cell carcinoma were irregular. The MVD were 3.97 ± 1.38,14.61±1.64 and 25.34 ±2.63 in PVC, LP and LC respectively, difference were statistically significant(P<0.01); There was a significant positive correlation between the expression of HPV16/E7 and Ki-67(r=0.854), the expression of Ki-67 was significantly correlated with the expression of MVD (r=0.878). Conclusion There was a significant positive correlation between The expression of HPV16/E7 and the proliferative activities and the malignant degree of ceels. Detected the expression of HPV16/E7 can be used to show the activated state of replication and transcription of HPV, the expression of HPV16/E7 can be served as another biomarker of cancerization relative to laryngeal papilloma. With a significant relation between tumor angiogenesis, cellular proliferation activity and pathological types The expression of Ki-67, MVD can be served as one of the malignant indicators for laryngeal papilloma in adults.
