Hepatoma is one of the hypsi-malignant digestive tumors in China, high fatality, poor prognosis, most of the hepatomas are developed from chronic B hepatitis and hepatic cirrhosis, so these diseases caused from hepatitis B virus have rather bad aspectsin patients'lives. chonic B hepatitis is a kind of chronic, diffuse diease, hepartic infusion and hemodynamics will change when hepatic cirrhosis occurs. Liver cirrhosis occurs when HBV attacked liver cells continiously, it's the common histologic change in many kinds of chronic liver dieases in advances stage, it's the final result. From above, early treatment on chronic B hepatitis and hepatic cirrhosis and early diagnosis will be crucial to improve the situation.Recently, with the development of ultrasound, especially the application of new method, we improved the diagnostic accuracy and got lots of precious diagnositic data. CDVP is a kind of new method lately, it can make use of the blood direction and speed with color coding, the faster blood flow rate, the brighter color is. It can not only describe the profile distribution of the blood flow rate, measure the transient blood of vessel section directly, but also tell the result automaticly, compared to the color Doppler, more accurately and conveniently. CDVP can measure the PVV and the direction of portal venous flow, it can reflect the pressure changes of portal vein. The study try to apply CDVP to detect the changes of portalhemodynamics analysis the liver damage, observe the changes of portal hemodynamics in HB and hepatic cirrhosis patients, so as to find the inner connection among them. we can diagnosis chonic B hepatitis and hepatic cirrhosis more efficiently.The study used Aloka Prosound SSD-5500 (made in Japan) with CDVP system, we divided the patients with chonic B hepatitis into four groups, including compensatory group (26cases), decompensatory group (29cases), chronic B hepatitis (29cases) and control group (31cases), we detected the main branch of portal vein, right branch of portal vein V and left branch of portal vein anteroposterio raxes V anteroposterio raxes inner diameter, portal vein D and splenic vein D, portal vein FV, splenic vein FV and portal vein V. The study shows, with the development of the hepatic cirrhosis, PVV all slower, while splenic vein V all faster, splenic vein FV increased gradually, the portal vein V blood flow increased in compensatory phase, meanwhile decreased in decompensatory phase. Right and left branch of portal vein blood flow all decreased in both phases.Compared to chronic B hepatitis group, in hepatic cirrhosis group, the blood flow rate of main portal vein, right branch and anteroposterio raxes of left branch went slower, PVFV increased, right anterior branches and left anteroposterio raxes blood flow reduced, splenic vein FV and splenic vein splenic vein V increased. Chronic B hepatitis group compared with the control group, the blood flow rateof main portal vein, right branch and anteroposterio raxes of left branch went slower, splenic vein V and splenic vein FV increased. Among the four groups, portal vein D and splenic vein D all increased gradually, the data in compensatory group increased more obviously than chronic B hepatitis group. CDVP can study on the portal hemodynamics, we can use it to know the portal hemodynamics when hepatic cirrhosis, then we can make good use of it to diagnosis and treat hepatic cirrhosis.CDVP is convenient, simple, objective, no wound, easy to receive, good reproducibility, it can offer exact, objective diadynamic criteria for chonic B hepatitis and hepatic cirrhosis, what's more, it can also be the basis of clinical treatment. In addition, we can diagnosis and treat the sub-clinical cases, measure the portal hemodynamics data, so as to estimate the course of LC. The study can reflect the portal hemodynamics in chronic B hepatitis or hepatic cirrhosis patients, it has great importance in discover the hepatic cirrhosis in early stage.
