objective:To explore molecular mechanism of cell apotosis of kidney tissue after severe burn with delayed fiuid resuscitation at different altitude. Method:A total of 240 male Wister rats were employed as the experimental models (TBSA 30%, III~°), and they were randomly divided into 3 groups delayed fluid resuscitation group(DFR), immediate fluid resuscitation group(IFR)and control group .The myocardium samples were harvested at 1,6,12,24,72 and 168h after burn respectively.The levels of HIF-1α,bcl-2,c-fos,Trail of cell in kidney tissue were detected by means of immunhistochemistry. Results:At 3840m altitude compared with each group in lanzhou area,The experession levels of HIF-1αprotein,bcl-2protein,c-fos protein in kidney tissue cell were much higher(P<0.001); and in DFR than those in IFR (P<0.001). At 3840m altitude compared with each group in lanzhou area,The experession levels of Trail protein in kidney tissue cell were decreased(P<0.001); in DFR than those in IFR (P<0.001). Compared with those of each group in lanzhou area, the apoptosis Index of cells (Tunel) in high altitude area increased . Compared with those of the IFR group, the apoptosis index of cell of the DFR group increased .Condusion:1,The pathologic changes of kidney tissues with delayed fluid resuscitation after burn in the rats at high altitude were more severe than those in lanzhou area.2,The kidney injury explicit happens in high altitude after Severe burn,and the injuries of the DFR group are much severe than those of the control group.3,Kidney injuries resulted from severe burn in high altitude area are relative to apoptosis of wreter cell.4,The expression of HIF-1α,c-fos,Trail protein play a role in promoting kidney cell apoptosis of severely-burnt rat in high altitude area,while the high expression of bcl-2 reflects the enhancement of self -protective mechanism in the course of injury.
