Objective:To investigate the effects of differently delayed resuscitation projects on rats' hearts and livers after scalded severely.Method:Eighty-five healthy SD rats of both gender at age of 23months,weighting 200g250g,were prepared for the study.The rats were randomly devided into 4 groups based on the time of resuscitation:?1?immediate resuscitation group?n=20?:after unhairing by 10%sodium sulfide,they were placed in hot water of 94? for 12 seconds to establish30%TBSA?total body surface area?of III degree scald model,and resuscitation treatment started at the time point of 0h after burned by enterocoelia infusion adding 1/3 of 24 hours total amount of fluid?according to the Parkland formula?;?2?delayed resuscitation group?n=20?:resuscitation treatment started at the time point of 6h;?3?no resuscitation group?n=10?,no resuscitation treatment;?4?normal group?n=5?,they were placed in water of 37? for 12 seconds with no resuscitation treatment.The rest of rats were randomly devided into 3groups based on differently delayed resuscitation projects:?1?accelerated delayed resuscitation group?n=10?:resuscitation treatment started at the time points of 6h,6h20min and 6h40min and added 1/6,2/6 and 3/6fluid of delayed resuscitation group respectively;?2?uniform delayed resuscitation group?n=10?:resuscitation treatment started at the time points of 6h,6h20min and 6h40min and added 2/6,2/6 and 2/6 fluid of delayed resuscitation group respectively;?3?decelerated delayed resuscitation group?n=10?:resuscitation treatment started at the time points of 6h,6h20min and 6h40min and added 3/6,2/6 and 1/6 fluid of delayed resuscitation group respectively.Blood and heart sample were collected at the time points of 1h,3h,7h,10h?immediate resuscitation group and delayed resuscitation group?,the time points of 7h and 10h?no resuscitation group,accelerated delayed resuscitation group,uniform delayed resuscitation group and decelerated delayed resuscitation group?,and the time points of 1h?normal group?after burned.Heart injury was investigated by levels of serum lactate dehydrogenase?LDH?,creatine kinase?CK?,creatin kinase-mb isoenzyme?CKMB?and histopathologic changes in heart slices.Liver injury was evaluated by levals of serum glutamic-pyruvic transaminase?ALT?,glutamic oxalacetic transaminase?AST?.Results:1.In every time points of the immediate resuscitation group,delayed resuscitation group and no resuscitation group,the values of LDH,CK,CKMB,ALT,AST were significantly higher than those of the normal group?P<0.05?.2.In the immediate resuscitation group,the values of CK,CKMB,LDH,ALT and AST were lower than those in the no resuscitation group at almost time points?P<0.05?.3.In the delayed resuscitation group,the values of CK,CKMB,LDH,ALT and AST were significantly higher than those in the no resuscitation group at the 7th hour?P<0.05?,and lower than those in the no resuscitation group at the 10th hour?P<0.05?.4.In the accelerated delayed resuscitation group,uniform delayed resuscitation group and decelerated delayed resuscitation group,the values of LDH,CK has no obvious difference with those in the delayed resuscitation group?P>0.05?.5.In the accelerated delayed resuscitation group,the values of CKMB were significantly higher than those in the delayed resuscitation group at the 7th hour?P<0.05?,and has no obvious difference with those in the delayed resuscitation group at the 10th hour?P>0.05?.In the uniform delayed resuscitation group,the CKMB values has no obvious difference with those in the delayed resuscitation group at the 7th hour?P>0.05?,and were lower than those in the delayed resuscitation group at7th hour?P<0.05?.In the decelerated delayed resuscitation group,the values of CKMB has no obvious difference with those in the delayed resuscitation group at every time point?P>0.05?.6.In the accelerated delayed resuscitation group,the values of ALT has no obvious difference with those in the delayed resuscitation group at every time point?P>0.05?.In the uniform delayed resuscitation group,the values of ALT were lower than those in the delayed resuscitation group at every time point?P<0.05?.In the decelerated delayed resuscitation group,the values ALT were lower than those in the delayed resuscitation group at every time point?P<0.05?.7.In the accelerated delayed resuscitation group,the values of AST has no obvious difference with those in the delayed resuscitation group at every time point?P>0.05?.In the uniform delayed resuscitation group,the values of AST has no obvious difference with those in the delayed resuscitation group at 7th hour?P>0.05?,and were lower than those in the delayed resuscitation group at 10th hour?P<0.05?.In the decelerated delayed resuscitation group,the values of AST has no obvious difference with those in the delayed resuscitation group at 7th hour?P>0.05?,and were lower than those in the delayed resuscitation group at 10th hour?P<0.05?.8.Pathology results showed that the myocardial injury of no resuscitation group was more serious than other groups,the injury was most obvious in the 10th hours after burned.The myocardial injury of the accelerated delayed resuscitation group,the uniform delayed resuscitation group and the decelerated delayed resuscitation group has no significant difference.Conclusion:1.Delayed resuscitation can cause the burned rats' heart and liver injury severer,and the reason of this phenomenon is that delayed resuscitation may cause the ischemia-reperfusion of the burned rats' hearts and livers.2.The uniform delayed resuscitation project is better than the accelerated delayed resuscitation project and the decelerated delayed resuscitation project to protect the rats' hearts and livers after severely burned.
