Objectives: we combined serum sex steroid determination, testicular volumemeasurement and testicular tissue biopsy to explore seminal neutral a -1 ,4-glucosid-ase activity changes in different causes of azoospermia. Our aim is to konw the diagnostic value measure seminal neutral a-1, 4-glucosidase activity in azoospermia and locate the obstructive Site .Methods: we choice 142 cases patients attendanced to the first affiliated hospital of Zhengzhou University because of male infertility. In semen analysis, no sperm is found, we do it for two or more times (test time interval≥15 days) . respectively we measured serum hormone (FSH, LH, PRL, T) and seminal plasma biochemical indicators(fructose, acid Phosphatase, a-1,4-glucosidase, neutrala- 1,4-glucosidase, WBC dyeing). In azoospermia patients, when unilateral testicular volume are above 12ml, FSH values are in the normal range, bilateral testicular tissue were biopsy, according Johnsen score to judgment spermatogenic function; Johnsen score= 8 is considered normal spermatogenesis ,which call obstructive azoospermia group( OA ); Johnsen score <8 is considered the spermatogenic obstacles azoospermia, which call non-obstructive azoospermia group( NOA ). NOA group have 80 cases, and OA group have 62 cases. Control group have 30 cases with normal Fertility. SPSS11.0 statistical packages for windows is used for the analysis of variance and correlation among the three groups. Several teams were used of two single-factor analysis of variance (Analysis of variance. ANOVA), a = 0.05, to compared three groups detection indicators whether have significant statistical significance or not.Results : Control group's seminal plasma a-1,4-glucosidase activity is 49.1±10.2mU/L, which is the highest in three group, non-obstruction group and obstructive group were reduced to 33.9±9.6 mU/L and 10.9±2.3 L mU/L. Obstruction group compared with control group and non-obstruction group have significant statistical significance(P=0.025;P=0.037). And the control group compare with non-obstructive group have no significant statistical significance(P=0.076).Control group's seminal neutral a-1 ,4-glucosidase activity is 26.2±6.5mU/L, which is the highest in three group, non-obstruction and obstructive group are reduced to 18.7±6.9 mU/L and 2.2±0.6 mU/L.Obstruction group compared with control group and non-obstructive group have significant statistical significance (P=0.019;P=0.023). But control group compared with non-obstructive group have no significant statistical significance .The ratio of seminal neutral a-1,4-glucosidase activity in the total a-1, 4-gl-ucosidase activity is 53.2% in control group . The ratio of seminal neutral a-1,4-glucosidase activity in the total a-1, 4-glucosidase activity is 49.2% in non-obstructive group. And the ratio of seminal neutral a-1,4-glucosidase activity in the total a-1, 4-glucosidase activity is 19.6% in obstructive group.The ratio of seminal plasma neutral a-1,4-glucosidase activity in total a-1,4-glucosidase decrease sequentially in three g-roup.Application Spearmann related analysis, we have the Conclusion that seminal neutral glucosidase activity has negative correlation with Jhonsen score, Bilateral epididymal size and bilateral testicular volume. FSH have negatively correlated with J-honsen score and bilateral testicular volume. We used testicular volume, serum FSH levels and neutral a-1, 4-glucosidase activity to diagnosis of azoospermia of 80 cases obstructive azoospermia and 62 cases with non-obstructive azoospermia, the diagnosis rate is 97.4% (78 /80) and 91.2% (57/62).Conclusions: In obstructive azoospermia, seminal neutral a-l,4-glucosidaseactivity have sensitivity change than total seminal a-1 ,4-glucosidase activity. In clinical, measurement of seminal neutral a-1,4- glucosidase in conjunction with other non-invasive examination have higher diagnostic sensitivity in azoospermia, it can avoid some patients from invasive inspection.
