Background and ObjectiveHenoch-Schonlein purpura (HSP) is one of the most common forms of vasculitis of childhood. The mechanism for the onset of Henoch-Schonlein purpura nephritis is unknown. Generally deemed that the type III allergy mediated by IgA and IgG participated the onset of HSP. But recently years clinical research found that type I allergy mediated by IgE maybe participated the partly onset of HSP. CD23 and CD19 are relates a closer factor with IgE. This article through detect the expression of CD23 and CD19 on peripheral blood lymphocytes in patients with Henoch-Schonlein purpura nephritis to investigate the role of CD23 and CD19 in pathogenesy of Henoch-Schonlein purpura nephritis, the interrelations between expressions of CD23 and CD19 on peripheral blood lymphocytes .Methods1. CD23~+ and CD19~+ expression on peripheral blood lymphocytes were detected by flow cytometry , and serum IgE was measured by using enzyme linked immunosorbent assay (ELISA) in 30 cases with Henoch-Schonlein purpura (HSP) and 30 cases of Henoch-Schonlein purpura nephritis (HSPN) and 30 cases health controls.2. Renal biopsy per cutem locating at B-type hyperacoustic was performed in 27 patients with Henoch-Schonlein purpura nephritis (HSPN). The nephridial tissue was examined at the light microscope,electron microscope and immunofluorescence.3. The data management and statistical treatment were undertaken by the statistics software SPSS 10. 0. Examination standard alpha=0. 05.Results 1. CD23~+ and CD19~+ expression on peripheral blood lymphocytes and serum IgE were significantly elevated in peripheral blood of the children with Henoch-Schonlein purpura(HSP) and Henoch-Schonlein purpura nephritis(HSPN) as compared with health control(P < 0. 01); CD23~+ and CD19~+ expression on peripheral blood lymphocytes and serum IgE were significantly elevated in peripheral blood of the children with Henoch-Schonlein purpura nephritis(HSPN) as compared with Henoch-Schonlein purpura(HSP) (P < 0. 01).2. There were 27 patients with Henoch-Schonlein purpura nephritis were strived for renal biopsy. pathology grades II level of 18 examples, III level and above 9 examples, The expression of CD23 and CD19 on peripheral blood lymphocytes in patients with renal pathology grades III and above level were significantly elevated as compared with II level(P < 0. 01).3. We found that the positive correlation between IgE,CD19~+ and CD23~+ was presented in peripheral blood of the children with Henoch-Schonlein purpura (coefficient correlation were 0. 322 and 0. 336, P < 0. 05 and 0. 01); but there is no correlation between IgE and CD19~+ (coefficient correlation was 0. 165, P > 0. 05).Conclusion1. CD23 and CD19 expression on peripheral blood lymphocytes and serum IgE were significantly elevated in peripheral blood of the children with Henoch-Schonlein purpura (HSP) as compared with health control, and both CD23 and IgE,CD19 were presented positive correlation. These results illustrated that CD23,CD19 and IgE possibly col-participated in the HSP occurrence and the development, those who possible play an coordinative or additive action.2. CD23 and CD19 expression on peripheral blood lymphocytes were significantly elevated in peripheral blood of the children with Henoch-Schonlein purpura nephritis (HSPN) as compared with Henoch-Schonlein purpura (HSP), and their diversities have statistical significance. And the pathology graduation higher, the expression of CD23 and CD19 on peripheral blood lymphocytes is higher, the difference has statistics significance. Therefore, to detect the lymphocyte CD23 and CD19 expression rate in peripheral blood of the children with Henoch-Schonlein purpura nephritis may take the reflection Henoch-Schonlein purpura nephritis renal pathological change serious degree target.3. CD23 may use in indirectly to appraise the immunity condition with Henoch-Schonlein purpura.4. Detecting CD23~+ and CD19~+ expression on peripheral blood lymphocytes and serum IgE are helpful on diagnosis and treatment of HSR But require to further study.
