Objective To study the expression of CXC chemokines IP-10,Mig and the relation ship between IP-10,Mig and HBV-DNA,ALT in the patients with chronic hepatitis B,and influence of IFN-αon their expression in peripheral blood.Methods The different levels of IP-10 and Mig in peripheral blood mononuclear cells(PBMC)of chronic hepatitis B were kinetically detected by real-time quantitative polymerase chain reaction(Real-time PCR)in different combination treatment stages 0,3,6 months of Interferon-α(IFN-α)and Ribavirin.The rate of Ig chemokine/lg GAPDH was regarded as the extreme level of chemokine;The plasma level of IP-10 and Mig were measured by enzyme linked immunosorbent assay(ELISA),and the expression of IP-10,Mig in hepatic tissue were deteced by streptavidin-peroxidase(SP) immunohistochemistry.Results The expressive levels of IP-10,Mig mRNA in PBMC,The IP-10 and Mig plasma level and the mean level of HBV-DNA,alanine aminotransferase(ALT)in peripheral blood of 45 patients were 0.7387±0.0768,0.6883±0.0693,(660.9±75.5) pg/ml,(609.6±73.8)pg/ml,6.07×10~5copies/ml,234.6U/L,respectively.It was higher significant difference in patients with chronic hepatitis B than that.in normal controls (P<0.001~P<0.05).In patients with chronic hepatitis B,the level of IP-10,Mig mRNA in PBMC were quite correlated with IP-10,Mig plasma level(r=0.7312,P<0.001;r= 0.7157,P<0.001),the IP-10 and Mig plasma level were fairly correlated,with the ALT plasma level(r=0.7235,P<0.001;r=0.7220,P<0.001),the IP-10 and Mig plasma level were extremely correlated with the HBV-DNA plasma load(r=0.7371,P<0.001; r=0.7266,P<0.001).The expression of IP-10 and Mig mRNA in PBMC were quite correlated with the ALT plasma level(r=0.4992,P<0.001;r=0.5740,P<0.001),The expression of IP-10 and Mig mRNA in PBMC were extremely correlated with the HBV-DNA plasma load(r=0.4379,P<0.005;r=0.4260,P<0.005).IP-10 and Mig were found to be expressed prominently on sinusoidal lining cells by immunohistochemical analysis.After combination treatment for three months,the expressive levels of IP-10 and Mig mRNA in PBMC,the IP-10 and Mig plasma level and the mean level of HBV-DNA and ALT were 0.5741±0.0912,0.5962±0.0987, (430.7±60.5)pg/ml,(415.4±65.8)pg/ml,1.30×10~4copies/ml,97.5U/L,respectively.So was higher significant difference(P<0.001~P<0.05).After combination treatment for six months,the expressive levels of IP-10,Mig mRNA in PBMC,the IP-10 and Mig plasma level and the mean level of HBV-DNA and ALT were 0.4235±0.0924, 0.4105±0.0898,(312.6±47.9)pg/ml,(309.8±52.3)pg/ml,3.42×10~3copies/ml,61.gU/L, respectively.There was higher significant difference than in normal controls (P<0.001~P<0.05).Conclusion This study shows that the expression of IP-10 and Mig are high in patients with chronic hepatitis B,and are positive associated with the level of replication of HBV and the level of ALT.IFN-αcan effectively restrain the replication of HBV and down-regulate the levels of IP-10 and Mig to adjacent,to proximal normal level in patients with chronic hepatitis B.IFN-αplays a key role in anti-viral and immune regulation during the treatment.
