Objective:To study the expression of IL-8(ELR~+) and IP-10(ELR~-) and their mRNA in peripheral blood and liver biopsy tissue of the patients with cirrhosis after infected by HBV,and the relationship between the level of ELR~+ chemokines and the expression of ELR~- chemokines,and the pathogenesis effect on cirrhosis,such as immune injury,infiltration of inflammatory.Methods:The typical patients with cirrhosis after HBV infection were selected, and their serum IL-8 and IP-10 concentrations were evaluated with ELISA,the contents of IL-8 mRNA,IP- 10 mRNA in peripheral blood mononuclear cells(PBMC) and liver biopsy were measured by Real-time PCR.The rate of lg cDNA/lg GAPDH was regarded as the extreme level of chemokine mRNA.The expression of IP-10 and its receptor CXCR3 in liver biopsy tissue was detected by SP immunohistochemistry stain.In the same time the expression and distribution of IP-10 and CXCR3 in PBMC and liver biopsy tissue were observed by laser scanning confocal microscope(LSCM), making use of fluorescence antibody to CXCR3,IP-10 labeled with FITC and Cy3, respectively.Results:The levels of IL-8 and IP-10 in serum of patients with cirrhosis after infected with HBV were(432.4±77.5)pg/ml and(299.3±61.2)pg/ml,which were significantly higher than those in controls(P<0.01),as well as the mRNA of IL-8 and IP-10 in PBMC with the level of(1.1134±0.2025) 1g cDNA/lg GAPDH, (0.7479±0.1495) 1g cDNA/lg GAPDH,respectively(P<0.01).In the same time the mRNA of IL-8 and IP-10 in liver biopsy tissue was(1.3651±0.2216) lg cDNA/lg GAPDH,(0.8563±0.1506) lg cDNA/lg GAPDH,respectively.There was significant difference between the patients with cirrhosis after infected with HBV and patients with surgical trauma in liver(P<0.01).There was significant correlation between IL-8 in serum and IL-8 mRNA in PBMC and liver biopsy tissue(r=0.567,0.498,P<0.01), as well as the level of IP-10 in serum with IP-10 mRNA in PBMC and liver biopsy tissue(r=0.464,0.557,P<0.05).The levels of IL-8,IP-10 in serum and IL-8 mRNA, IP-10 mRNA in PBMC and in liver biopsy tissue with HBV-DNA(+) in serum was (462.6±91.2)pg/ml,(342.3+80.3)pg/ml,(1.2212±0.2207) lg cDNA/lg GAPDH, (0.8465±0.1528) lg cDNA/lg GAPDH,(1.5028±0.2207) lg cDNA/lg GAPDH, (1.0024±0.1517) lg cDNA/lg GAPDH were higher than those in serum of the patients with HBV-DNA(-)(P<0.01,P<0.05).There was significant correlation between IL-8, IP-10 in serum,IL-8 mRNA,IP-10 mRNA in PBMC and in liver biopsy tissue and the load of HBV-DNA in serum(r=0.636,0.796,0.703,0.750,0.633,0.620,P<0.01).And there was also significant correlation between IL-8,in serum and the level of ALT(abnormal),AST(abnormal),ratio of AST/ALT(>1.0)(r=0.543,0.542,0.507, P<0.05 or P<0.01),as well as IP-10 in serum,IP-10 mRNA in liver biopsy tissue with them(r=0.630,0.578,0.648,P<0.01)(r=0.639,0.686,0.704,P<0.01).Moreover,SP immunohistochemistry stain showed that the expressions of IP-10 and its receptor CXCR3 were upregulated,which were concerned with the infiltration of inflamed cells. CXCR3 mostly distributed in the header of hepatic tissue,the region of fibroplasia,the region of hepatonecrosis and IP-10 mainly express on sinus hepaticus endothelial cells. With this stain IP-10 took on reinforced phenomenon in patients with HBV-DNA(+) or aminopherase abnormal,which was also confirmed by the technique of laser scanning confocal microscope(LSCM) making use of fluorescence antibody to CXCR3,IP-10 labeled with FITC and Cy3,showing green and red,respectively.Conclusions:The levels of IL-8,IP-10 and their mRNA in peripheral blood and liver biopsy tissue of the patients with cirrhosis after infected with HBV were all upregualte,which have close correlation with the duplication of HBV-DNA and degree of injury in hepatocyte.IL-8 and IP-10 play key roles in the local inflammatory infiltration of liver and the progression of posthepatitic cirrhosis.The expression of IL-8 and IP-10 maybe has dynamic equilibrium in peripheral blood and local liver tissue of the patients with cirrhosis.
