Purpose:To investigate the clinical efficacy and influences of blood rheologyand interleukin-6(IL-6) of electroacupuncture on Xi-cleft points with Jiaji pointscuring lumbar intervertebral disc prolapse(LIDP). To set standard for the clinicalevaluation of this disease through the quantification of symptom and physical sign, and at the same time, to develop the corresponding experimental research and todiscuss blood stasis type of LIDP's mechanism of action on needling Xi-cleft pointsby copying inflammatory models of LIDP's nerve root, in order to offer the clinicexperimental proof.Method: (1) In clinical research, sixty selected cases were randomly dividedinto treatment and control groups, 30 cases in each. Treatment group:Electroacupuncture on L4 to S1 Jiaji Points, Waiqiu, Weizhong, Jinmen and otherspecial acupoints with rarefaction-dense wave, 30 minutes one time, 5 times per week,4weeks as a period of treatment: Control group: Routine acupuncture treatment, 5times per week, 4 weeks as a period of treatment. Curative effect could be evaluatedby quantifed scores of symptoms and physical signs, and pain visual analoguescale(VAS). Synchronously, changes in IL-6 and blood rheology, which appearedbefore and after treatment were observed. (2) method: 24 rat models with chemicalradiculoneuritis were founded in order to mimic LIDP(which were randomly dividedinto acunpuncture and model groups and 12 models in each), and blank control groupwas 12 ones, too. The acupuncture group was treated by needling local Jiaji points,Xi-cleft points in Foot-shaoyang channel and Foot-taiyang channel and Weizhong, and there were no treatments for.the model and blank control groups. In that cases, there were changes in the behavior science of rat models and neurocyte morphologyto observe, inflammatory filter paper's weight, analysis of rheology of blood, nitrousoxide(NO), IL-6 and other inflammatory factors to mensurate. Results: (1) After a period of treatment, the scores of treatment and controlgroups were 18.47±10.46, 22.67±10.9; 3.47±1.45, 5.67±1.52, according to evaluation of symptom and objective sign quantitized scores, and pain VAS scores. The curativeeffect of the two groups was P=0.012 with Ridit method or P<0.001 with t-test. Everyindex in IL-6 and blood rheology of the two groups had been improved and the t-testof the two groups was P<0.001.After60 patients with LIDP were treated by treatment, the evaluation of the curative efficacy with quantized scores of symptom felt byyourself, physical sign, quality of life and others, compared with evaluation of the onewith the Lumbago scoring proposed by Fairbank JC, and the result was that theformer was better than the latter.Experimental study was to observe the influences ofrat models with chemical radiculoneuritis on needling the Xi-cleft points, and todiscuss mechanism of action of needling on LIDP. (2) The acupuncture groupimproved activities of the rat models' affected bodies, promoted the recovery of painand touch senses, ameliorated all gists of blood rheology and nerve root'sdegeneration, lightened inflammatory reaction and reduced the level of inflammatoryfactors(NO,IL-6), which were all better than the model and blank controlgroups.(P<0.05).Conclusions:in the treatment group, cinical efficacy and the improvements onsymptoms, physical signs, daily life ability and raising leg straightly were better thanthe control group. Quantized scoring method including symptoms, physical signs andquality of life can regard as one of the objective methods on evaluating LIDP'sefficacy. In rat models, acupuncture can improve affected body's movement andnerve's function, lighten inflammatory reaction and ameliorate nerve root'sdegeneration. The mechanism of action might be connected with decrease of NO andIL-6's levels, and the flowing speed of the blood rheology, which could eliminateinflammation of nerve root and reduce inflammatory algogenic factors.
