Clinical Observation And Experimental Study On Lumbar Intervertebral Disc Herniation With The Combined Therapy Of Jiaji Electro-acupuncture And Lumbago Granules

ObjectivesThe purpose of this project is to observe clinical efficacy of electro-acupuncture Jiaji points combined with lumbago granules for patients with lumbar disc herniation,and to explore the mechanism by testing the inflammatory factor in L5nerve root of rats witch were transplanted autologous nucleus pulposus and also treated by electro-acupuncturing Jiaji points combined with lumbago granules. To provide a more effective treatment for the patients with lumbar disc herniation on clinical.Method1Clinical Study:60cases lumbar disc herniation patients from the Acupuncture Department outpatient and inpatient department of Wuhan No.1Hospital meeting the diagnostic criteria and inclusion criteria were collected, and were randomly divided into treatment group (Electro-acupuncture Jiaji points combined with lumbago granules) in30cases and observation group (Routine electro-acupuncture combined with lumbago granules) in30cases. Treatment group were selected bilateral Jiaji points corresponding to the segment of intervertebral disc protrusion, ipsilateral Huantiao, with ipsilateral Yanglingquan when it is L4-5intervertebral disc herniation and Weizhong when it is L5-S1intervertebral disc herniation. Observation group were selected Dangchangshu、henshu、 Yaoyangguan、Weizhong、Ashi point as main points. Two groups were given to lumbago granules for2weeks.2experiment study:40male SD rats were randomly divided into5groups:Electro-acupuncture Jiaji points group, lumbago granules group, Electro-acupuncture Jiaji points combined with lumbago granules group,model control group, blank control group, with8rats in each group.24rats were established transplantation of autologous nucleus pulposus model. The model control group only exposed the L5right nerve root without placed nucleus pulposus, one week after operation, the treatment was started after wound healing,Jiaji electro-acupuncture group was acupunctured bilateral Jiaji points corresponding to L5and L6nerve roots at a depth of5mm, connecting with HANS electro-acupuncture instrument in continuous wave,0.2mA,30min; Lumbago granules group was given lumbago granules after dissolved; Electro-acupuncture Jiaji points combined with lumbago granules group was treated with Electro-acupuncture Jiaji point combined with lumbago granules gavage in the above-mentioned method;Model control group was given cool boiling water, and no treatment given to blank control group.All the treatments were once a day for2weeks. The heart perfusion was carry out in the end of the experiment, pick the L5nerve root tissue, and then detect the content of iNOS with immunohistochemistry method.Result 1Clinical study:Among30patients in the treatment group,3cases were cured,21cases were Significant effective,5cases were effective and1case was invalid. Among30patients in the observation group,1cases was cured,15cases were Significant effective,11cases were effective,3cases were invalid. The effective rate of treatment group was96.67%, and the observation group was90%.2Experimental Study:The experimental result shows that the positive expression of iNOS was observed in the nerve roots. Compared with the model control group,the IOD value decreased significantly in all lumbago granules group, electro-acupuncture Jiaji points group and electro-acupuncture Jiaji points combined with lumbago granules group(P<0.01). It indicated that two kinds of treatment methods could reduce levels of iNOS in nerve roots. Electro-acpuncture Jiaji points group and lumbago granules group had no significant difference (P>0.05), and compared with the electro-acupuncture group and lumbago granules group, the IOD levels of electro-acupuncture Jiaji points combined with lumbago granules group decreased significantly(P<0.05).Conclusion1Clinical study:The result showed that in clinical treatment of LDH electro-acupuncture Jiaji points combined with lumbago granules was more effective than conventional acupuncture combined with lumbago granules.2Experimental Study:Those indicated that it was more significant for Jiaji electro-acupuncture combined with lumbago granules on decreased the levels of iNOS in nerve roots.
