Clinical Research On Inflammatory External Hemorrhoids With Fuzhi Powder

ObjectiveHemorrhoids are one of the most common and frequently encountered diseases of clinic. Inflammatory external hemorrhoids are common emergency cases in anorecal surgery with sudden onset. It brings patients great pains, which is the main cause for visiting or hospitalization. The treatments of inflammatory external hemorrhoids roughly include conservative treatment and operation at early stage. Operation can not be accepted in that it will bring damage to tissues, causes postoperative pain and edema and even leads to recurrence. With the development of pathogenesis, anatomy, physioloy and pathology of hemorrhoids, people know its therapeutic purposes is to relieve symptoms but no eradicate hemorrhoids. Therefore, anorecal surgeons pay more attention to conservative treatment.In this study, we base on clinical practice, use the special Traditional Chinese Medicine(TCM) powder named "fuzhi powder" to treat hemorrhoids. It contacts directly with lesions, which has the effect of clearing away heat evil and promoting diuresis, activating blood circulation to dissipate blood stasis, subduing swelling and stopping pain, restrainting water and arresting bleeding. It aims at improving local blood circulation and lymph circulation, promoting absorption and dissipation of inflammation. We try to find new effective formula of external application for conservative treatment of inflammatory external hemorrhoids in TCM through the study on fuzhi powder.Methods In this research, we assigned randomly patients of inflammatory external hemorrhoids to the treatment group or the control group. In the treatment group, "fuzhi powder" was used, while in the control group, "Mayinlong ointment" was used. Before application (8h each time, twice each day) sits bath with potassium permanganate solution(1: 5000) were both used. To compare the effect between the two groups, common symtoms and signs of hemorrhoids such as pain and dragging sense of anus, volume change and mucosa change of hemorrhoids were analyzed on 3^rd day, 5^th day and 7^th day after 7 days' treatment.ResultsThere are 50 cases in the study with 25 cases in each group. Comparing results of general condition for balance between two groups:Sex: P=0.771, Age: P=0.901, Course: P=0.738, Pattern of syndrome: P=0.799, P＞0.05 of all results, There was no statistical difference. It means that the two groups possess comparability in general condition.1. Comparing results of treatment in symptoms and signs between two groups:(1) Pain between two groups:Before treatment, P=0.472＞0.05. There was no statistical difference. After treatment P_d3=0.000, P_d5=0.002, P_d7=0.000, P＜0.05 of all results. There was significant difference. It means that the treatment group was superior to the control group in effect in relieving pain.(2) Dragging sense between two groups:Before treatment, P=0.875＞0.05. There was no statistical difference. After treatment P_d3=0.04, P_d5=0.04, P_d7=0.03, P＜0.05 of all results. There was significant difference, lt means that the treatment group was superior to the control group in relieving dragging sense.(3) Volume change between two groups:Before treatment, P＞0.05. There was no statistical difference. After treatment P_d3=0.02, P_d5=0.03, P_d7=0.02, P＜0.05 of all results. There was significant difference, lt means that the treatment group was superior to the control group in decreasing swelling of hemorrhoids. (4) Mucosa change between two groups:Before treatment, P=0.861＞0.05. There was no statistical difference. After treatment P_d3=0.04, P_d5=0.04,P_d7=0.03, P＜0.05 of all results. There was significant difference, lt means that the treatment group was superior to the control group in protecting mucosa.2. Comparing results of total effects between two groups:The total effective rate of the treatment group was 96%,which had remarkable difference with control group(P＜0.05).Conclusions1. "fuzhi powder" can reach local lesion by external application, which has the effect of clearing away heat evil and promoting diuresis, activating blood circulation to dissipate blood stasis, subduing swelling and stopping pain, restrainting water and arresting bleeding. It can improve local blood circulation and lymph circulation;promote absorption and dissipation of inflammation.2. "fuzhi powder" is good at relieving pain and dragging sense of anus, decreasing swelling and protecting mucosa than "Mayinglong ointment" (P＜0.05).3. The total effect of "fuzhi powder" is better than that of "Mayinglong ointment" (P＜0.05).4. There are no significant side effects and local dermato-allergic reaction after external application of "fuzhi powder". It is safe and effective in treating inflammatory external hemorrhoids.5. Treating inflammatory external hemorrhoids with "fuzhi powder" has the advantages of little pain, short course of treatment, high effective, easy application and cheap expense. It is an effective formula for inflammatory external hemorrhoids.
